(Is your child/adult with developmental disabilities being told they have to

WAIT for services such as personal support, day services, supported

employment, natural support, special medical supplies, etc.?)

4. EPSDT (Early Periodic Screening Diagnosis and Treatment)

(Is DCH denying your child treatment for a condition which should be covered

under federal Medicaid law?)

5. Approval of the NOW Waiver.

(Would you like Georgia to allow more flexibility and self-determination in

their MRWP waiver services? Would you like to know why the NOW Waiver (written

and approved by DHR) has STALLED in DCH? Would you like the NOW Waiver to be
