pertussis?

Around Oct. 7, Brennan (9, cvid, asthma) had an apparant asthma flare. Slight

runny nose, no temp, terrible, harsh coughing fits that did not get better with

neb tx. On third day, took him to after hours clinic, they put him on pred for a

week, and with rest, round the clock tx and pred he improved. Tuesday this week,

the exact same symptoms returned. No symptoms, then suddenly, harsh, unrelenting

cough that is not improved with tx.

Wed was IVIG. Since the cough had started overnight and worsened so quickly and

sounded so tight, his nurse called his ID dr. who came down, agreed something

was up, but thought pulm should handle it, bc its in the lungs, so passed it to

pulm office. Our pulm was out of town and the on call was supposed to come down,

but became too busy so sent a nurse down to check on him. They were supposed to

call in something. They didnt. Wanted to wait and see if he improved over the

weekend (yeah, right).

So just got home from pcp and she says pertussis is going around the area, cant

tell for sure, but is starting him on abx and pred in case it is, bc some of the

symptoms fit.

Great!! he isnt making the whooping sound, but he is coughing so hard it catches

his breath and makes his " brain " hurt. Tx is not improving it at all.

None of my kids have ever had pertussis. Not knowing for sure makes it

hard...like how long do I keep him home? should I contact family with babies we

were in contact with last week?

Has he had level checks since on IVIG? He should have good immunity through his

infusions and actually be better covered than most kids. Could be a change of

season lung flare up.

That is good to know, thanks! Nick finally seems to be getting better. This is

day 18, two rounds of antibiotics, one round oral steriods and I think finally

upping his Advair dose is what is making him better.

Amy, sorry I'm late to this discussion (I'm going back to school and have too

much on my plate.) Just wanted to share that, last year, there was a pertussis

scare in my area. Our immuno tested my daughter's titers (she is on scig) for

pertussis and her levels were inadequate. We were told that she does not have

protective levels, even on IgG replacement. However, the doctor said that there

is probably enough to prevent a major catastrophe -- but not enough to stop her

from acquiring the bug. If there is a strong suspicion of pertussis, I can see

why your doctor suggested antibiotics as this is the cure.

Nick has a cough that is just getting worse and worse. We just took him to the

immuno yesterday and they ran some tests but told us that it couldn’t be

whooping cough since he is on IVIG.
