I'm now almost through with two weeks of vancomycin again. This seems like a stubborn case, so my doctor is going to keep me on four vancomycin a day for two more weeks, then three a day for a week, two a day for a week, and one a day for a week. I think someone on here posted that he was given that tapering off treatment when he had it. Has anyone on here had one or more recurrences? The bummer is, that I just had fat put into my face recently, and since I got sick with this I've dropped about 15 lbs. It kind of goes from the face first. I had a patient with several recurrent bouts of C-Dif. The man, about 45, was also low in body fat, and has both HIV and HCV, so his immune system is always busier. Several 2 week Vanco treatments didn’t work, and he was also losing weight he could not stand to part with. I had discussed the case Sherwood Gorbach, MD, editor in chief of Clinical Infectious Disease) about what else might be done for this patient. Gorbach warned to not over-treat with Vanco, as this will too much disrupt the gut flora, needed for colon cell repair. He suggested using Bismuth (Pepto Bismol) after the Vanco if needed, for bouts of diarrhea and cramps. When people are taking Vanco for C Dif., I urge them to take 10 grams (1 Tablespoon of powder) L-glutamine, twice a day. This is important lymphocyte fuel for the cells of the small and large intestine. I also have them take Culturelle, probiotic, twice a day. Many docs around here suggest Florastore (saccharomyces bulardi). The Florastore company seems to have some data that gives their product a slight edge over Culturelle in C. Dif therapy. However, there was a poster at CROI this spring, detailing nice GALT (Gut-associate Lymph Tissue) T cell re-population in HIV+ people taking Culturelle. (Lactobacillus GG).
