Guest guest Posted May 14, 2012 Report Share Posted May 14, 2012 do u feel that cobicistat by itself is too worrisome in terms of kidney damage? or is it the combo of cobicistat and tenofovir together? i myself have been anxiously awaiting the Prezista all-in-one with GS7340. i'm presently on Prezista, Norvir, and Truvada and i don't think i'd want to be on cobicistat plus viread, but was hoping that cobicistat plus gs7340 would be a wash compared to norvir and viread. Guest guest Posted May 14, 2012 Report Share Posted May 14, 2012 We know that boosted PI + Truvada regimens cause higher decreases in creatinine clearance than non boosted combos. But Cobicistat is not what we hoped as a boosted to replace Norvir. Cobicistat may have a stronger negative influence in the presence of tenofovir than Norvir did. And the interactions it may cause with other drugs are complicated. I would not switch to QUAD from Atripla. Having said that, a switch from Atripla to Isentress plus Truvada has shown great efficacy and safety. But isentress needs to be taken twice a day, which may not be a big deal for some people having side effects from Atripla who may want to switch away from that once a day pill. Boosted Prezista plus Truvada will be the next one pill a day combo after QUAD. After that, we will have dolutegravir plus Epzicom. So, in 3 years we will have 5 once a day one pill combos. 5 years from now we will have a once a week combo. 10 to 15: a cure made up of a combination approach. I just polished my crystal ball. I hope I am alive to see all that become a reality. Guest guest Posted May 15, 2012 Report Share Posted May 15, 2012 ,i assume u already know that the Prezista-combo pill will not contain Truvada, as the Viread is being replaced by a new pro-drug of Tenofovir currently labeled "GS7340". i also assume u know that Gilead is testing a separate formulation of the elvitegravir-"QUAD" replacing tenofovir with GS7340 (that newer version is in Phase 2 currently).do u feel that the claims of reduced nephrotoxicity for GS7340 vs Tenofovir r overstated?i gather that norvir + tenofovir is less nephrotoxic than cobicistat + tenofovir......but what i really hoped u n the list might know (or have an opinion on) is whether or not norvir + tenofovir is more or less nephrotoxic than cobicistat + GS7340....any idea?
