How old is your child with Autism?

When was he/she diagnosed?

When did you see behaviors?

What were the first behaviors that made you question Autism?

Does he/she have other siblings with Autism or another Neurological

disorder?

Does he/she have issues with sleep? What kind? How much sleep does

he/she get on average?

What other gut issues does your child have?

What other diagnoses does your child have?

Is your child a picky eater? If so, what kinds of things will he/she

eat? What will he/she NOT eat?

What are his/her biggest issues now (social, aggression, sensory, etc)?

What therapy's/treatments have you tried? Have they helped?

What therapy's/treatments would you like to try? Why don't you (money,

insurance)?

Have you had relationship issues since dealing with Autism? Have you

divorced/separated in this time?

What do you think caused YOUR CHILD'S Autism?

Any other comments, etc...you'd like to add....
