Any of you with any degree of kidney disfunction should be aware that acyclovir (and probably Valtrex, too, since they're related) crystalizes in the kidneys which may damage your kidneys further. If acyclovir or Valtrex is necessary to treat a herpes or shingles outbreak, dose reduction may be necessary. If you are using acyclovir to prevent outbreaks of shingles and/or herpes, you may want to weight the benefit of outbreak prevention against the risk of further kidney damage. If your HIV is undetectable, your CD4 count is good and you don't have frequent herpes outbreaks in the absence of acyclovir prophylaxis, then I wouldn't continue acyclovir prophylaxis in the presence of kidney damage.
