Most people switched a few years ago due to pill burden and metabolic issues. I am assuming that this is your first and only combo. Atripla has an advantage that is one pill a day, but it can affect sleep and fatigue in a small proportion of patients. Although no study has determined exactly what factors predict these side efffects, it seems that people with history of depression or substance abuse may have more problems with Atripla. It also seems that African Americans attain higher blood levels of Efavirenz and thus may have more side effects. Lipids may also go up, but not as much as with the combo you are on, Boosted Reyataz is also once a day with Truvada. It has none of the central nervous system effects that Atripla has but around 20 percent of people can develop jaundice due to high bilirubin. The combo is three pills once a day. I've been on the same drug regimen for the last 16 years and my new doc had given me the option to change from Indinivir/combivir/acyclovir yo either reyataz, sustiva, or tuvada. I have neuropathy and my feet are completely numb otherwise healthy and my current meds are working but it would nice to go down to 1-2 pills pet day. Guest guest Posted November 10, 2010 Report Share Posted November 10, 2010 Dave,There are so many individual variables in how one responds to various drug combos in treating HIV, you really have to work with your Doc. I had a change in regimen about 2 years ago from Trizivir--which was so simple--to one with Sustiva, Lexiva, Norvir and Isentress. I had to drop the Lexiva and switch to Reyataz because of really high cholesterol and triglyceride (522) levels. Apparently Norvir also boosts lipid levels, and there was a recent study (where?) showing no apparent difference in efficacy between boosted and non-boosted Reyataz--tho' that was for treatment naive patients. I am cautious about dropping the Norvir without more solid information. As for Sustiva: I continue to have bizarre dreams almost every night. Some are totally weird, and have me singing in my sleep or sitting up in bed watching my curtains turn into origami butterflies and the smoke alarm light becoming a giant illuminated skull. Not nightmares--just totally weird. My former HIV doc thought the weird dreams would go away in a few months, but two years later I still have them, which I do not find pleasant. I have not experienced any neuropathy under my current or previous regimen. To cope with the increased lipid levels, my primary care physician recommended I take pravastatin, and double the amount of fish oil, Co Q-10 and niacin (the flush-free type) I was taking.
