I think NYC is closer to Philly than Pittsburgh, but if you want to stay in PA,

the UCSF HPV/Anal Cancer/Dysplasia Clinic lists one in Pittsburgh:

The website cited, at UCSF Mt. Zion's Anal Neoplasia Clinic is not updated frequently; better to call their office manager, Fred Fishman at 415.353.7443 for the most current list of specially-trained provided with the equipment to perform the procedure....
