, everytime you put on electrodes you must have a ground electrode.

Has nothing to do with the design.

Somewhere on the head, doesn't matter where.

On the body of the client.  Preferably on the head.  Preferably somewhere about equidistant from the active electrodes.  With snap-ins I use the back of the neck below the hairline.  With cup electrodes, Cz or center of the top of the forehead are good spots.
