I live alone. I go out alone. I eat alone. I think alone. I'm so much on my own that I'm nearing mental collapse. I live in a foreign country and I do not share my HIV status with anyone around me. There are no support groups for English-speaking folks. I'm at a loss! Suicidal thoughts are looming on the horizon and everything seems dark and dull. Even my family act as if they feel for me, but the truth is they do not get it. They have no idea how I'm feeling inside. Huge Drop in CD4 Count! Should I Start Meds?I was diagnosed in December 2007, and my CD4 count was 565 in February 2008. However, it's dropped very quickly since then, and was 388 as of my last test in November 2008. My doctor thinks this drop is rather unusual and wants me to start HIV meds soon. Is he right? Does this large CD4 drop mean that, when I do start meds, they may not be as successful? A Blip or Not a Blip?I've been on Atripla (efavirenz/tenofovir/FTC) since I was diagnosed three years ago. Three months after starting Atripla, my viral load became undetectable, and it had been that way ever since -- until Nov. 4. That was when my doctor told me that my viral load was now 150, but that it was only a "blip." Do you think I should be concerned?

I tested negative three months after the exposure, but now I'm sure my health is going downhill. Is there still a chance I have HIV, or possibly hepatitis C? If it was an HIV-positive person that he gave oral sex to, could there have been HIV in my friend's mouth, and could I then have gotten HIV from him when he blew weed smoke into my mouth? I brushed my teeth before we started and he claims he rinsed his mouth out after the oral sex, but I don't believe him.

What is the life expectancy of someone diagnosed with HIV at age 60? I am 60 years old and I became HIV positive around the middle of 2008. My latest CD4 count was 622 and my viral load was 3,490.

 Multivitamins and HIV

What's your opinion about vitamin supplements such as K-PAX Immune Support Formula? I'm 28 years old and I was diagnosed with HIV in October 2008 with a CD4 count of 690.

 Long Work Hours and Aging With HIV

I'm 56 years old and have had HIV for six years. My viral load has been undetectable for most of that time and my CD4 count is around 1,000. I have two questions: One, do HIV-positive people over 50 still work or do most go on long-term disability? And two, will long work hours harm me over the long haul? I work a rotating 12-hour shift with some occasional 18-hour shifts.

 Does Steroid Use Affect the Immune System?

I was a steroid user until August; I mostly did cycles of Deca-Durabolin (nandrolone decanoate) and Sustanon (a mix of testosterone compounds). I found out six weeks ago that I was HIV positive. Can taking steroids compromise my immune system further? Will they affect my HIV medications when I start taking them?

0Treatment After a Break: Which Meds Will Work?

I've been on a drug holiday for the last six and a half years. My CD4 count is now down to 385 and my viral load is in the 1,500 range. I think it’s time to begin HIV treatment. How can I know what medications will still work against my HIV?

 CD4 Count Not Moving Up After Years of Treatment

I've been HIV positive since 2005. I started out with a CD4 count of 80. It climbed into the low 200s by 2006, but since then it has been hovering in the 230s with occasional spikes into the low to mid 300s. My viral load has remained undetectable while on my current regimen of Norvir (ritonavir), Reyataz (atazanavir) and Truvada (tenofovir/FTC). I've spoken to two HIV specialists, and they have opposite views on whether I should change my meds to try to raise my CD4 count. What do you think?

 Long-Term Problems With Bio-Alcamid

Three years ago I got Bio-Alcamid (poly-Alkyl-Imide) injections in my cheeks. Now, one area has become a bit red, swollen and painful, and the filler has also crept up into the corner of one of my eyes. What should I do?

I bought a bottle of body lotion from Walgreens but didn't open it for a few days. When I finally did, I noticed the lotion seemed especially thick on the top. My friend insisted that I shouldn't use the lotion; he says some people open up lotion bottles and put semen in them to try to infect people with HIV. Is this true?

How Can a Baby Born With HIV Test Negative Two Years Later?I was just watching The Tyra Banks Show. A woman on the show said her mom adopted a child that was born with HIV, but then tested HIV negative at the age of two. How is this possible? Any Advice for Someone Newly Diagnosed With a Low CD4 Count?I'm a male in my mid-20s. I was diagnosed with HIV on June 2, 2009. For most of the month, I experienced symptoms common for seroconversion. I met my infectious disease specialist today -- my CD4 count is at an apparently troublingly low level of 240, and my viral load was 130,000. I have yet to take any HIV meds but my doctor prescribed Atripla (efavirenz/tenofovir/FTC). I'm trying my best to keep my head up and all, but my lab results are scaring the hell out of me. Any general advice or pointers?

HIV TREATMENT

Using Isentress in an Innovative Way to Lower Viral LoadI was infected and diagnosed three months ago. My CD4 count is 500 and my viral load is over 500,000, which I've heard is normal for a new infection. I decided to start meds earlier rather than later after weighing the pros and cons. I'm on Atripla (efavirenz/tenofovir/FTC) and Isentress (raltegravir). My doctor is hopeful that I can wean off Isentress after my viral load comes down. Is this strategy normal? How soon can I expect my viral load to come down? CD4 Count Low: What Multivitamins Do You Suggest?I was diagnosed with HIV in January 2009 and my CD4 count is 238. I was told that my CD4 count is not low enough to start treatment and that I should just take care of myself to try and boost it. I'm currently taking vitamin C and vitamin B complex. Is there any other supplement you'd suggest I take, besides what I'm currently getting from the clinic? How Can You Have Resistance to a Drug You've Never Been On Before?I am 22 and I was recently diagnosed with HIV. The specialist got the resistance tests back and it turns out that both my fiancée's HIV and mine have drug resistance to NNRTIs (non-nucleoside reverse transcriptase inhibitors, or "non-nukes"). This changes our treatment options. How does this resistance occur when I've never taken any HIV medications before? Also, I need to take something to prevent PCP (pneumocystis pneumonia) but I'm allergic to Bactrim (co-trimoxazole) and dapsone (Avlosulfon) so I have to have intravenous treatment for PCP. Is this just as effective?

SIDE EFFECTS & OTHER HEALTH ISSUES

Sexual Dysfunction After HIV DiagnosisI was diagnosed HIV positive at the end of March. I have been taking Atripla (efavirenz/tenofovir/FTC) for almost a month with no side effects that I can discern. My problem is one of erectile dysfunction that has manifested since my diagnosis. I'm a 60-year-old married man; my wife, who is HIV negative, is very supportive and we have an excellent relationship and a healthy sex life. Before this, I was like the Energizer bunny, able to almost get an erection on demand; now every attempt at intercourse just ends as an exercise in frustration. What can I do about this problem? My wife says it's in my head, and I tend to agree.

Is it likely that the depression will disappear soon as I get used to Atripla? Should I look to change to another HIV combo or increase my dose of citalopram? Can Marijuana Help Decrease HIV Med Side Effects?I've noticed that smoking marijuana greatly reduces the side effects associated with my HIV medications. Is this common with other HIVers? I casually mentioned this to my doctor and even inquired about obtaining it for "medicinal" purposes. My doctor wasn't sure that it was indicated for medicinal purposes in HIV-positive people and dismissed the topic. Do you think it is indeed indicated in HIV disease, and if so, what channels would I take to get it? I am in Canada, and the rules may be different from those in the U.S. Do You Think Retin-A Would Be Good for Residual KS Scarring?I had Kaposi's sarcoma on my leg that has been clearing up with HIV treatment, but it appears to be a really slow process. Could Retin-A (tretinoin topical) cream help this process along?

When Might Tesamorelin Be Approved for Fat Accumulation in HIV-Positive People?My belly is big with intra-abdominal fat without subcutaneous fat. I know that the only treatment for it, tesamorelin, is close to approval and that an application was submitted to the U.S. Food and Drug Administration on June 1. If it is found to be safe and effective, when might it be approved? Are there still any studies on it in which I would have a chance of getting the medication (instead of the placebo)? Fillers for Lipoatrophy Around the Eye?I've been experiencing facial wasting and the place I have been losing more fat than anywhere else on my face is under my eyes. My plastic surgeon told me he doesn't treat the area around the eyes in the case of lipoatrophy because it's "too dangerous." Is this true -- even with Sculptra (poly-L-lactic acid, New-Fill) and Radiesse (calcium hydroxylapatite, Radiance) injections? Is there anything that can be done? When Will I See the Full Results of My Sculptra Treatments?I had five rounds of Sculptra (poly-L-lactic acid, New-Fill) four years ago. It did pretty well but now I need a touch up. Is There Such a Thing as HIV Immunity?I am a 43-year-old gay male and I have been having sex with men for around 25 years. I know this is not something to put lightly, but I have had unprotected sex with at least 80 men. I even had a lover of five years who was HIV positive and we never used condoms. I am NOT bragging, but wondering why and how I am still HIV negative. I know what I am doing is wrong, but could there be something about me that keeps me HIV negative? I take an HIV test every six months at my local health department and it has always come back negative. Is It Recommended to Use a Latex Condom With a Female Condom?Would it be safe for me to wear a well-lubed condom (say, lubed with silicone oil, which does not dry out like water-based lubes) in conjunction with my girlfriend using a female condom? Or does this scenario share the same drawbacks as double-bagging (i.e., using two condoms)?

Can You Get HIV From Nose Secretions Mixed Into Food?A friend of mine who is HIV positive was eating spicy food, and his nose was running. He used his left-hand finger to clear his nose, and used the same left hand to take the curry from the bowl with a spoon. Afterward I used the same spoon to take curry. If his nose fluids fell into the curry bowl and I consumed some of it, would I be at risk for HIV? Should I get tested? Please answer, I am very worried over this incident.

I did weight training for a good 12 years, but as of a couple of years ago, I train rarely because I have neither energy nor motivation. I have muscle loss on my arms, abdomen and legs. It is very depressing. What can I do? I don't want to take too many supplements. Is there a protein shake you would recommend?

HIV TREATMENT

Is It Normal to Still Have Problems With Atripla After a Year? I have been on Atripla (efavirenz/tenofovir/FTC) for one year. My viral load is undetectable and my T-cell (CD4) numbers have hovered around 300. What's troubling is that I have been experiencing severe anxiety and depression, including thoughts of suicide. I have also been having increased crazy dreams and dizziness. Can it be that for some reason the Sustiva (efavirenz, Stocrin) in the Atripla is starting to cause these symptoms after months of things being OK? I have had a history of anxiety and depression, but never bad enough to warrant going on medication for it. When I have had these symptoms, they have come out of the blue, with no outside life issues prompting them. Any advice?

HIV DRUG RESISTANCE

Has My Virus Become Resistant to Isentress?I have had HIV/AIDS for 25 years and I have become resistant to most HIV medications. My latest regimen has been Isentress (raltegravir) and Truvada (tenofovir/FTC). I recently went through an "on and off" phase with my regimen because I moved to a different state. I have had nothing but infections since then. Does this mean I have drug resistance? Resistant to All PIs and Almost All NRTIs: What Are My Options?I have been HIV positive for 25 years and have done pretty well on HIV medications, which I began about 17 years ago. Last year my viral load began to increase so I switched my regimen. My phenotype and genotype tests showed that my virus was resistant to all protease inhibitors (PIs) except Prezista (darunavir) and all nucleoside reverse transcriptase inhibitors (NRTIs) except Viread (tenofovir). I showed no resistance to any drugs in the non-nucleoside reverse transcriptase inhibitor (NNRTI) class. My doctor put me on Norvir (ritonavir), Prezista, Isentress (raltegravir) and Atripla (efavirenz/tenofovir/FTC). I have allergies to sulfa drugs and after being on Prezista (which contains sulfa) for one year, I've been dealing with the rashes and itching because it is the only PI that I am not resistant to. However, the itching is getting unbearable. I have had three doctor opinions now and all have concluded that it is a drug reaction. I was hoping for your opinion before I talk to my doctor next week about stopping the Prezista. My viral load is currently undetectable and my T cells (CD4 cells) are at 303. My ratio is 30.3 percent. Am I getting to the end of the road for my options?

Found Out June 24, First Test Results Next Week(A recent post from the "I Just Tested Positive" board) I got an HIV-positive result on June 24. I saw a specialist on July 3 and I get my first test results next week on the 16th. I'm 56 and I am totally overwhelmed by this -- I cry without warning, have anxiety attacks, little interest in food. I'm feeling deeply ashamed and embarrassed. I live in a pretty small town that is OK with gay people but I don't think they can handle this. My husband/partner/best friend of 26 years has been great and I've told our one other sex partner (so far both are negative). Over the July 4th holidays we had lots of family around and everyone could tell I was "not OK" but I couldn't talk to anyone about it ... I still know almost nothing. I'm feeling very lost and afraid and find it hard to focus on anything. Luckily I'm a runner and still doing my three miles every few days -- it seems an OK way to clear my brain for a short time anyway. ... I'm hugely worried about test results next week. My prior HIV test (the last negative one) was 18 months before. Anyone out there like me? Is My CD4 Count Drop Normal?I was diagnosed HIV positive in October 2007. At the time, my viral load was 6,310, my CD4 count was around 600 and my CD4 percentage was 28 percent. I've had a few tests since then and the results have been consistent. But in the five months between my two most recent tests, my CD4 count went from 780 with a percentage of 32 to 539 with a percentage of 30. I am not on HIV meds yet. Is this a "normal" drop in CD4? Is There a Way to Manage Viral Loads Without Meds?My first viral load test result was 30,791 and my second result was 37,000. I'm not yet on HIV medications, but I am a healthy eater and a positive thinker, plus I exercise and take some supplements. What else would you recommend I do to manage my viral load? Is there a magic workout formula, food or supplement that would help?

What Is PEP?This may be a stupid question, but what is "PEP"? Can't Get Tested for HIV Because I Would Be Deported If PositiveI live and work in a country where foreigners are deported if they test HIV positive. I recently visited another country and intended to get tested there but I chickened out at the hospital. I told the doctor I was having pale stools (which is true) and asked if I could get a liver function test and urine test to check my digestive system. I had a blood test and urine test and the results suggest no big problem. My question is: Do these results mean that I should not be worried about HIV? My possible HIV exposure was six months ago.

Are There Interactions Between HIV Meds and Omega-3 Fish Oil or Flaxseed?

I have borderline high cholesterol. I'm taking Kaletra (lopinavir/ritonavir) and Truvada (tenofovir/FTC). I take omega-3 fish oil and flaxseed with each meal two to three times a day. I also take my HIV meds in the evening about 45 minutes after dinner. Are there any interactions between these two supplements and my HIV meds that I should worry about?

Doctor Told Me I'm a Long-Term Nonprogressor -- Can This Be True?

My first test results came back and were very good. My HIV specialist asked me what my nationality was. When I told him Scots Irish he told me that makes sense to him. Evidently many people with northern European ancestry have a defective gene that makes it more difficult for HIV to get a firm hold on the body. (The same gene evidently makes you more susceptible to the West Nile virus). My question is how many of you have been given this glimmer of hope titled long-term nonprogressor, only to be disappointed later when it turns out to not be true? My doctor commented on my lack of emotion to the news. I just can't bring myself to put any hope in his theory this close to the original diagnosis.

Anyone out there like me? Being in a rural place is hard and I could use some people to e-mail with.

Is Atripla and Isentress a Good Combo? Is It OK to Miss Doses?

I am on Atripla (efavirenz/tenofovir/FTC) and Isentress (raltegravir) after being on Trizivir (AZT/3TC/abacavir) for seven years. During my last five years on Trizivir, I had a viral load of 5,000 but my CD4 count continued to climb from 200 to 1,000. Should I not have switched? After one month on Atripla and Isentress, my viral load was undetectable. After two more months we dropped the Isentress. Six months later my viral load climbed to 30,000, though my CD4 count was still about 1,000. We added Isentress again and three weeks later my viral load is 820=2

0and my CD4 count is 1,044. However, I have been missing doses at least once, sometimes twice a week. Is this a problem?

Should My 6-Year-Old Be on HIV Meds?

My 6-year-old daughter was diagnosed with HIV eight months ago. I adopted her when she was only 4 days old, and she was diagnosed when the doctor was looking at something else totally unrelated. She is in great health. Her CD4 count is 1,130 and her viral load is 4,300. I have a wonderful doctor for her who is saying that it's OK not to start medications until her viral load and CD4 count start to get worse. Do you agree?

How Could My CD4 Count Have Fallen So Rapidly?

I tested HIV positive in December 2008. My first CD4 count was 820 and my viral load was 17,000. I did another test in March 2009 -- my CD4 count was 808 and my viral load was 4,500. In June I tested again -- my CD4 count had fallen to 468 and my viral load had gone up to 62,000. I was n

ot sick -- not even with a cold. How could this have happened?

How Low Can My Viral Load Go?

My doctor didn't mention anything to me about that, so I doubt he's aware of it (there's no warning on either of the leaflets that come with the medications). My question is: Do you think the interaction is substantial? Should I be halting my Propecia intake or just monitoring it?

I am engaged to a wonderful man who happens to be HIV positive. We have been going out for three years and I love him dearly. He is doing really well (the numbers just keep getting better at every six-month visit and he is symptom-free, not counting the side effects from his meds). It isn't something we talk about much and it doesn't really affect our day-to-day life, besides our being far "safer" than the average couple. However, he is very private about his status, so none of the people we hang out with know.

Sometimes I just feel alone and would like to connect with others who are in a "mixed" relati

onship. One day we would like to have children, and I would love to hear from those who have done this. I also have a few questions like "How do I lessen my anxiety about having sex?" I am extremely ashamed of my anxiety, and would love to hear that I am not alone and that there is a way for me to work through this so we can both enjoy each other more without the stupid anxiety.

OI Meds, Overwork, Viral Load Blip: What Could Be Causing My Fatigue?

I am a 49-year-old gay male. I avoided getting an HIV test for years, but in spring of 2008 what I thought was just the flu was actually several opportunistic infections (OIs), including MAC (Mycobacterium Avium Complex). I spent an entire month in the hospital. I was put on Atripla (efavirenz/tenofovir/FTC), as well as clarithromycin (Biaxin) and ethambutol (Myambutol) for MAC and other drugs to prevent or treat OIs. My viral load dropped to undetectable within a few months and my CD4 count has been climbing, though I was told I was very anemic and had low platelets. When I went off the MAC drugs, I had a little bit of extra energy but I am now experiencing fatigue more than usual. My last test showed that my viral load went up a little bit. Could this be causing the fatigue? I also went back to work full time and I'm on my feet a lot. Could the fatigue come from working too much?

Is It Normal for My Boyfriend to Get Sick a Few Months After Getting HIV?

My boyfriend was diagnosed with HIV on May 20, with an exposure date of April 8 and textbook symptoms two weeks after exposure. He doesn't have medical insurance and gets ADAP (AIDS Drug Assistance Program) care through the Los Angeles Gay and Lesbian Center, but he's not on medication because no baseline has been established. Due to budget problems, his appointments keep getting pushed back. He had been very healthy (though he smokes moderately) until just a few days ago when he started feeling some congestion and fatigue. Last night he told me that his body was starting to ache all over. Could these be HIV-related side effects? Is it common for recently diagnosed HIVers to fully go through seroconversion and then get sick again later? What should we do?

At What Point After Taking PEP Should I Get an HIV Test?

I am currently on a course of PEP (post-exposure prophylaxis) to prevent possible HIV infection after a risky encounter. When is the earliest time I can have an HIV test? He had a good job with the government for over 25 years, but within a year of splitting up with my mother, he got hooked on crack cocaine. Within two years, he had lost his job, house, car and over $500,000 in savings. He's been totally reckless with his health. I've been pushing him to see a doctor for years. Three weeks ago he finally went in. He's lost a ton of weight, has a persistent fungal skin infection all over his body, in his mouth, and recently has trouble even getting out of bed. Yesterday he called me after getting his blood test results. Even though I knew in theory that he was likely HIV positive, I was devastated to hear he's been diagnosed with "full-blown" AIDS, as he put it. I'm afraid he won't even bother to seek more help or medication from the HIV clinic at the hospital. I don't know what the point of this post is, aside from needing to "vent." What can I do for my father?

I inhaled some scentless farts recently. I know inhalation is a route of medicine administration, so things you inhale do get into your bloodstream. Finding a Regimen That Doesn't Cause Body Shape Changes

I have been on a drug "holiday" for the past 18 months, after 12 years of HIV therapy. I stopped my meds with a high CD4 count of 1,100 and an undetectable viral load. I stopped therapy even though I felt great, because I had significant facial wasting and redistribution of fat to my abdomen. Now, after 18 months off treatment, my CD4 count is 450 and my viral load is 50,000, so the vacation is over. My doctor has recommended Epzicom (abacavir/3TC, Kivexa), raltegravir (Isentress) and either maraviroc (Selzentry, Celsentri) or etravirine (Intelence). Is this combo relatively free from fat wasting and fat redistribution problems? I hate to seem vain, but quality of life is as important to me as lab values.

Can Someone Recently Diagnosed With HIV Have a Normal Life?

I was diagnosed with HIV four months ago. I believe I was infected eight months ago maximum. My numbers are fine -- CD4 count of 542 and viral load of 10,000. My CD8 count went from 550 to 750, 35 percent to 41 percent. I'll see the results from my resistance test next week. Meanwhile, I wanted to ask you: How normal will my life be with this infection? Will it really involve only taking one or two pills a day or is my life going to change dramatically because of HIV medications, side effects, etc.?

Just Diagnosed at Age 41 With CD4 and Viral Load Dropping

It's ironic that I work in HIV. I have kept myself negative for all my 20s and 30s until a condom broke and I was unable to get PEP (post-exposure prophylaxis). My CD4 count dropped from 489 to 389, but my viral load also dropped from 38,333 copies to 33,000. My question is: When should I begin taking HIV medications?

HIV-Positive Recent Graduate Entering the Workplace

I just graduated college and found out I was HIV positive. I am currently on no medications, but I do see a doctor for labs and such. This is covered by my current insurance through my parents. However, this insurance will expire in March 2010. I am currently unemployed and am seeking a job. I'm somewhat concerned because of the lack of positions in my field and wonder if I am going to need to apply to Starbucks or Home Depot, which offer general insurance for employees, to keep coverage. The other option is to pay the COBRA fees for my parents' insurance ($800 a month) or just rely on White. I noticed there is a pre-existing condition clause that applies for about 60 days when transitioning between jobs. Does this also apply when switching from parents' insurance?

Do I Have End-Stage HIV?

At my last doctor visit my viral load was 150 and my CD4 count was 170. My doctor says I have end-stage HIV. I am not sure what that means. My CD4 count has never gone above 170 since I was diagnosed with AIDS. I don't know what to expect from here on. Is this the end? My T-cell (CD4) count has been as low as 59 but it's back up to 170 -- is that good? Please help!

Viral Load Blips: How High Can a Blip Go?

For around four and a half years while on Atripla (efavirenz/tenofovir/FTC), my viral load has been undetectable. Then four months ago (May 2009) it went up to 2,920. But as of a few days ago (September 2009) it's gone back to undetectable. I felt very discouraged after that detectable result, as I've never missed a dose, not even once, although sometimes my dosage times vary by a few hours. I was tested for resistance in 1998 and it was found that I had 184V, the one for 3TC (Epivir, lamivudine). I'm pleased that I've returned to an undetectable viral load, with a CD4 count now at 655, but I'm worried about whether this viral load blip is presaging some sort of weakening of my HIV therapy.

My Boyfriend Found Out He's HIV Positive and I'm Nervous About Sex

My boyfriend recently found out he's HIV positive. He says he was infected about six months back when he knowingly had unprotected sex (this was before I met him). He anticipated that I would leave him, but I really do love him and have chosen to stick with him. However, whenever we're about to have sex, I start getting scared and lose my erection. We've educated ourselves on the precautions of being a "magnetic" couple," but I know that sex will never feel 100 percent safe. I start getting scared a condom will break or that he'll have cuts in his mouth. It's affecting our relationship. Am I just being paranoid or is this a legitimate response to his HIV status?

Semen on an Irritated Anus: Do I Need PEP?

I recently had sex with a guy (mid-20s, unknown status). We were lying in bed with my ass quite close to him and he was jerking himself off. No penetration. I got some of his ejaculate on my anus. Seconds later I realized the skin around my hole was irritated and (possibly) cut. A bit of his semen could have slipped into the rim of my hole or membrane. How would you describe my risk? I went to hospital about 20 hours later and lied about a slipping condom to be given post-exposure prophylaxis (PEP).

How I Beat Cigna: A Note About Long-Term Disability Cancellation

This is a remark on a recent "Hot Topics" question someone asked about his/her long-term disability benefits being cancelled by Cigna. After receiving long-term disability benefits from Cigna for nearly 15 years, I received a letter from them stating that I was no longer considered disabled. I was dumbfounded, as I had been deemed disabled in 1994, and had been receiving social security along with Medicare benefits since that time. I contacted an attorney who advised me to include in my appeal letters from friends and family about how they have seen my health deteriorate over the years. I also included every detail of my HIV/AIDS-related health issues and how they affect my wellbeing. With the economy going in a downward spiral, it is my belief that insurance companies such as Cigna are looking for loopholes in records to get out of paying long-term benefits. To make a long story short, I am happy to say my appeal was successful. Not only were my benefits paid retroactively, but they ended up offering me a lump-sum settlement. Cigna cannot begin to fathom the mental distress of a person living with this disease.

Sculptra or a Fat Transplant: Which Is Better for Treating Lipoatrophy?I have very noticeable facial wasting. I'm considering either Sculptra (poly-L-lactic acid, New-Fill) treatments or having fat transplanted from my abdominal area to my face. Which treatment would be most effective -- and which is most affordable? Could Combining Facial Fillers Cause Painful Side Effects?I had silicone facial filler placed in my nasal folds many years ago. Then, about four years ago, I got Bio-Alcamid (poly-Alkyl-Imide) injections in my cheek near the side of my nose where the silicone was placed. Now I have a hard knot inside the end of my nose, and the skin in that area feels painful and tight. My sinus area is also swollen, but not red or hot to the touch. Could I have an infection? Could there be a problem with the two fillers interacting?

However, I'm looking for a regimen that won't affect my brain, since I've experienced increased forgetfulness and loss of concentration over the past year. I've read that Isentress has low penetration of the central nervous system, but I've found very little information about the central nervous system and the Prezista + Norvir combo. Why Use Condoms for Oral Sex?If the risk of HIV transmission from giver to receiver of oral sex is so miniscule, then should anyone really bother with condoms in these situations? Where should we draw the line between "risky" and "not risky" when it comes to oral sex? Oral Sex vs. Penetrative Sex While on HIV Meds: Equal in Transmission Risk?Dear Dr. Bob: It seems from some past posts that you don't consider unprotected oral sex in a magnetic relationship a dangerous activity, yet you caution magnetic couples to use condoms every time they have penetrative sex -- even if the HIV-positive partner in both cases is taking HIV meds. If the risk of transmission from both activities is considered extremely low, why do you continue to recommend condoms only for intercourse?

How Do Experts Determine the "Average" Lifespan for HIVers?

I've known my HIV-positive status for about 15 months. I recently read about a study showing that HIVers will probably live about 20 years less than their HIV-negative counterparts. I know the study results don't necessarily apply to me personally for a large number of reasons. But I also hear about all these long-term HIV survivors who end up with liver, kidney, heart and brain issues that can shorten their lives. I'm starting to worry that I might not see my 50th or 60th birthday. What do you think about the lifespan issue?

Now That My CD4 Count Is Up, Can I Stop My HIV Meds?

In 2002, I started HIV medications when my CD4 count was lower than 200. Now my CD4 count is over 500 and my viral load is undetectable. However, these tablets make me feel as if I'm high all day; my focus, memory and balance have gone haywire. I think a lot about stopping my meds completely. What do you think?

Headaches and Rash on Atripla: What Do I Do?

I have been on Atripla (efavirenz/tenofovir/FTC) for about seven weeks. I initially had no side affects besides a strange dream or two. But moving into week four, I have had to take Advil twice or more a day because I get the worst headaches. I'm also feeling sleepy more often than ever before and I regularly suffer from muscle fatigue. Lastly, I have a minor, non-itchy rash from my collarbone to my knees. Is there anything I can do to treat these side effects, or do I need to switch regimens?

Can I Eliminate One of the Meds From My Regimen?

I am a 45-year-old, HIV-positive man and I'm quite fit and healthy. I started taking Isentress (raltegravir), Kaletra (lopinavir/ritonavir) and Truvada (tenofovir/FTC) when my CD4 count was only 100 and my viral load was very high. My CD4 count is now 500 and my viral load is undetectable. I've been on this regimen for a year with no big problems. Now that I'm doing better, can I stop taking the Kaletra? If I did this and my viral load went up, could I go back on it at the same dose I now take?

Does Testosterone Replacement Really Help People With HIV?

I have been taking testosterone replacement treatments (most recently Androgel) for many years to treat the negative effects of HIV and HIV meds. However, my new doctor recommends against testosterone replacement: He says it is ultimately bad for HIV-positive people because the risks are too great and there are no benefits. This is completely opposite my prior doctors' advice for the past 10-plus years. Who is right?

A Teething Baby Bit Me! Did I Put It at Risk?

About a month ago my 8-month-old nephew bit my face while playing. He is currently teething and has razor-sharp teeth. After the incident, I checked my face and there was a small (pinhead-sized) amount of blood on my face. Although a part of me says, "Don't worry, it was a tiny bite, you are in treatment with stable counts, and there was an insignificant amount of blood," the other part of me is very concerned about the incident and a possible HIV exposure for my nephew. Should we have him tested?

 How Can I Get My Doctor to Prescribe Nandrolone Along With Testosterone?

I've been HIV positive for at least 23 years and have had the same doctor for all this time. Over the last five years my quality of life, body fat level and non-immune blood work (cholesterol, sugar) have all been less than optimal. I slowly started making changes -- quitting smoking and drinking, changing my diet, taking supplements, finding a personal trainer and, eventually, starting testosterone replacement therapy. I've seen great improvements in all areas, but I have concerns about my doctor's ability to properly monitor the testosterone therapy -- he was hesitant to put me on it in the first place; and he refuses to consider adding nandrolone decanoate (Deca-Durabolin), without explaining why. What do you think could be going on?

 Could Periodic Regimen Changes Help Keep Side Effects at Bay?

I started taking Sustiva (efavirenz, Stocrin) and Truvada (tenofovir/FTC) less than a month ago. My CD4 count is over 650 and my viral load is low. I'm aware of the side effects of my current regimen and am planning on switching the Truvada in favor of Epzicom (abacavir/3TC, Kivexa) once my viral load is undetectable. What do you think of the notion of changing regimens every six months or so, for the sole purpose of lessening potential side effects? Are there any advantages or disadvantages to this plan?

I developed acute retroviral syndrome and became very ill for two weeks; I couldn't work and had to go to the emergency room three times. My viral load was greater than 10 million, while my CD4 count was 330, so my doctor started me on meds. Ten weeks after starting, my viral load is undetectable and my CD4 count is 500. Still, did I start treatment too soon?

My husband wasn't anemic when he got his first HIV lab test results. We went for our second test after four months and his CD4 count has gone up from 78 to 180. However, now the results show he has macrocytosis, which I believe has something to do with anemia. Is this serious? Is it related to him taking HIV meds?

I've had a long and varied sexual history. I discovered the world of cruising in public restrooms, parks and gay saunas when I was 16, and became HIV positive when I was 21. Thinking about and having anonymous sex never ended, even now that I have a loving partner. I masturbate or search for sex online even during work hours. I've never been faithful. It's time and money consuming and it threatens my professional life, my relationship and my health. Most of the time I can fake a mask of " it's just that I really enjoy sex " ; but the fact is I'm governed by sex, and I feel stuck. What can I do?

I've Got a Crush and It Hurts So Good: How Can I Put Myself Out There?

I'm in my mid 30s, single and have no friends. There is this library that I go to, and I have had my eyes on this guy who works there as a library assistant. I have secretly admired him since he started working there years ago. He seems very sweet, and all I can do is think about him. All I can think about is: What are the negative things that can happen if I tell him? I am older than him, HIV positive and he is probably not even gay. I have not been in love for a very long time and have been celibate for many years. But here comes this beautiful stranger and I am all vulnerable again; I love this feeling but I also don't like putting myself in this situation. Has anyone else ever had a crush like this, or had trouble getting out there and dating? I'd appreciate any advice you have to give! -- joejoe5673
