Hi asked me to respond to you since I've been through the exact same thing myself. I wouldn't worry about it being too private to post--many of us have posted our experiences on the list, and it helps to trade info.

You're lucky that you're at UCSF with Berry & Palefsky--they're the tops in the field. With your low t-cells, I wouldn't wait to continue treating this. The lower your t-cells, the more likely you are to get cancer. I'd treat it now, and hope that your t-cells go up so you're less likely to have problems. Effudex is pretty easy to apply, and can help keep it from becoming cancer. It can cause some minor irritation, but it's pretty tolerable. If you do get cancer, you might end up with a colostomy and have to go through chemo and radiation, so I'd do everything you can to avoid getting to that point. I'll share my experience--I had warts operated on, which came back and turned out to be cancer and required two more surgeries to eliminate. I used Effudex & aldara in the interim. After that my doc would prophylactically apply podophyllin (a kind of mild acid) on the whole rectal area to burn off the to layer and prevent recurrences. I've been cancer and high grade dysplasia-free ever since, and didn't have to go through radiation or chemo. Hope this info helps. They have a number of studies on this they're doing at UCSF, so you might consider one of those as well. There's a topical cidofovir study that should be open now. I have a question that is too personal to post. I have been treated for the past year for high and middle grade anal dysplasia (warts) at Mr Zion (UCSF- Dr. Barry) with no success and a lot of pain during and post treatment. My CD 4's dropped to 66 and a biopsy came back high grade. He wants me to start putting effudex there, 5 days on, 9 days off for the next 3 months until I seee him again. Do I need to be that worried if I choose not to go ahead with this treatment? I am having no symptoms. Can I wait until my t's go up before I have to do this?

The treatments suggested are just not all that bad, and the disease is terrible.JB
