It may be for all of Illinois (maybe even other states since it mentioned the National Association of Counties), but when I received the card it says " Lake County " on it. It gives you a discount if your insurance doesn't cover a med. It is FREE. You just fill out a form and mail it in. I don't know where to apply, but here is the phone # for " members " , maybe call and ask them for a form, or Google some of the info below: 1-877-321-2652 card says Lake County then there's a logo for NACO, the National Association of Counties on back is logo for Caremark it also says: " This plan is not insurance " It saved us $57 for the whole family when Tamiflu wasn't covered by our insurance. Get it!!! (mom to , age 7, dairy intolerant-related GERD -- currently has polysaccharide antibody def, previously had transient IgG, IgA, t-cell & other defs... and also to Kate, age 3!, more dairy intolerant but very healthy!)
