Does the vomiting get worse as the day goes on? Does he have issues with constipation at all? What meds is he on? It sounds like his stomach may not be emptying fast enough. Reglan (and other meds like Erithromycin and Bethanechol) will help the stomach empty faster and therefore reduce the amount of spitting up. If he is constipated, that can further back up the system. There is a test called a Gastric Emptying Study that can dx a delayed gastric emptying time. It may be worth looking into. I would recommend seeing a ped GI if you are not already. Reflux can come in 2 ways...silent and obvious. Silent tends to be worse than the obvious reflux since it can do damage that you are not even aware of since you do not see vomiting. The more obvious form has visible vomiting, sometimes to the point of being projectile. It is possible for a child to change from one to the other. Age tends to change symptoms as well. My son was a vomiter for the longest time and then one day he stopped completely. He still has reflux, but it is now silent and much harder to pinpoint. Even with a fundo, he still, at times, refluxes past the wrap. We know this to be true from a number of Endoscopies that show damage to his esophagus and stomach fundus. Hope this helps. I would definitely look into further testing to see if it is reflux or not, but it sure sounds like it to me. Hope this helps and good luck! I think the dx of reflux is kinda a BS one.. has GI bleeding with this.. but with reflux why can he drink 24 ounces of neocate right after the vomiting and go to bed...?????????? have you heard of this..????? It is very possible that your son eats so much to help ease the pain of the reflux episodes. Also, the formula and milk they drink tends to be thicker and coats the throat and helps soothe the stomach as well. I have reflux myself and I usually try to eat a bowl of ice cream before taking an antacid if I have any acid breakthrough episodes. I know it may not seem like a possiblilty to you, but it sounds a lot like reflux to me. I would certainly not rule it out until something more diagnostic can be done to determine something more concretely. Do they have any plans to test or scope to try to determine why he is having these issues? I know with my son, he was on very high doses of Prevacid and even that was not enough, so he had to take Zantac and Prevacid concurrently to have adequate acid control. I have the same problem....I need both as well. Both of us do still occasionally have acid breakthrough episodes even on that amount of medication. Guest guest Posted August 19, 2004 Report Share Posted August 19, 2004 2 full scopes have been done.. minor irritation, of the esosoghus, on pepcid and prevacid, and mylanta, for mouth pain. I have severe gerd as well, going to have surgery for it.. When making a Dx you need to compute all the factors involved.. He has more than just simple nightly vomiting, he has GI bleeding still? HIgh ALT liver counts, abnormal metabolic tests, not conclusive to any specific pattern for a metabolic disease... as well as low IGa and IGG.. factoring in the intolerances to all food except oatmeal... and only with pancreatic enzyme.. I feel suspicious that a dx of reflux is that easy... given all the meds... he has been on since birth.... I agree that he is a comfort eater and with the volume of neocate he comsumes a day 60-70 ounces he just made the 50 percentile.... I hope that they are not overlooking something like a pancreatic problem or EE.. I DX patients myself, so I am leary of a quick fix.. She would throw up every single night for 2 - 3 weeks, stop for a week and then start up again. The doctor said that he thought it was reflex (not reflux). She had started out with a terrible stomach virus, but it became reflexive. We gave her some anti-spasmatic meds with her bottle 3 or 4 times a day and it stopped. But everytime she has a tummy virus, she throws up more than normal and we usually have to go back to the anti-spasmatic to calm it back down. She's fine the rest of the time. (She is the same one with EBV, but I don't think the problems are related as she had the tummy problems well before the outbreak of blisters in her mouth and throat.) Of course, this conclusion was reached after all of the tests for reflux...... You might keep track of if there is a pattern that involves a particular food in case allergies are involved.
