I sit my son sideways on my lap and lean him back about 45 degrees. I spray then lay him back all the way and tip his head back. We sing 3 short songs then he can get up. It took him a few days but the learned the routine real fast. Sometimes we are in a hurry and he does get the spray standing up. I've noticed if I put the sprayer too far up his nose it makes a weird noise and it all runs back out. If I only go halfway in I get a nice mist type sound from the spray and not a single drop comes out.
