Here are the questions we used in the survey:

1. Screen name

2. Gender

3. Age

4. Enthicity

5. Country of residence

6. Age when first diagnosed with hypertension

7. Age when diagnosed low potassium, if applicable

8. If you suffer from low potassium, when it was first discovered were you given a reason for it being low? Reason given?

9. Do you require potassium supplements now? If yes, how much do you take?

10. (Females) Did you have high blood pressure before you were pregnant?

11. Age when Primary Aldosteronism first diagnosed by a physician

12. Type of physician who diagnosed your PA

13. How many physicians did you see before you were finally diagnosed with PA?

14. Cause of your PA: Hyperplasia Unilateral adenoma Bilateral adenomas

15. What were your main symptoms before you were diagnosed with PA?

-unexplained tiredness or lethargy despite sleeping well at night

-heart palpitations or irregular heart beat

-poorly controlled blood pressure despite multiple meds

-brain fog or feeling of confusion

-getting up at night to pee

-muscle cramps, tremors or twitches

-headaches

-numbness or tingling of extremeties

-episodes of unexplained sweating/flushing/feeling hot

-muscle weakness

-unexplained chest pain

-unexplained thirst

-metallic taste in mouth

-unexplained feeling of anxiety

-other

16. Are you still bothered by any of these symptoms, despite treatment for PA?

17. Since your diagnosis and treatment for PA, would you say that most of the time you feel: generally well generally unwell

18. Have you had AVS? (Adrenal Vein Sampling)

19. If you answered "Yes" to the above question: where done? done properly w/ ACTH? both sides sucessfully done?

20. Current dose of Spironolactone/Inspra

21. Has a physician ever recommended the DASH diet as part of your treatment for PA?

22. Do you measure your blood pressure at home? If no, why not?

23. Pertaining to your blood pressure:

What was your highest blood pressure before receiving a diagnosis and treatment for PA? What was your last measured blood pressure?

24. Have you had an adrenalectomy?

25. If yes to above question, when was it done? Where was it done? Did you have AVS beforehand? Have any symptoms recurred?

26. Have you ever been told you had any of the following? glucose intolerance or insulin resistance? Left ventricular hypertropy? Damage from high BP when a doctor looked inside your eyes?

Other Conditions related to HPTN?

27. Before your diagnosis of PA, how many times per night did you get up to urinate?

28. Relating to the above question: how long going on? more urine night than day? Spiro/Inspra make it better? Did surgery make it better? If yes how much better?

29. Questions related to your kidneys. If you answer yes to any question, please explain cause and amount, if known. Have you ever: had blood in your urine? had albumin in your urine? had kidney stones? had a kidney infection? had a prostate problem? (male)
