Age when first diagnosed with hypertension 7. Age when diagnosed low potassium, if applicable 8. If you suffer from low potassium, when it was first discovered were you given a reason for it being low? Reason given? 9. Do you require potassium supplements now? If yes, how much do you take? 10. (Females) Did you have high blood pressure before you were pregnant? 11. Age when Primary Aldosteronism first diagnosed by a physician 12. Type of physician who diagnosed your PA 13. How many physicians did you see before you were finally diagnosed with PA? 14. Cause of your PA: Hyperplasia Unilateral adenoma Bilateral adenomas 15. What were your main symptoms before you were diagnosed with PA? -unexplained tiredness or lethargy despite sleeping well at night -heart palpitations or irregular heart beat -poorly controlled blood pressure despite multiple meds -brain fog or feeling of confusion -getting up at night to pee -muscle cramps, tremors or twitches -headaches -numbness or tingling of extremeties -episodes of unexplained sweating/flushing/feeling hot -muscle weakness -unexplained chest pain -unexplained thirst -metallic taste in mouth -unexplained feeling of anxiety -other 16. Are you still bothered by any of these symptoms, despite treatment for PA? 17. Since your diagnosis and treatment for PA, would you say that most of the time you feel: generally well generally unwell 18. Have you had AVS? (Adrenal Vein Sampling) 19.
