Guest guest
I had AVS twice and didn't have to do anything special to prepare each time. Kept my normal work, exercise, crazy busy life schedule up to the day of the procedure, and was back to normal about 2 days after the procedure each time. To be on the safe side, I waited a week before any strenuous exercise or stretching after the AVS. As for wait between AVS and surgery - my second AVS was in early August IIRC, and my surgery was in mid-October. Would have liked it to have been sooner, but between the wait for the AVS results to be interpreted, and the schedule of my chosen surgeon, that's how it worked out. I was unmedicated and fine the whole time. One thing to keep in mind is that if you go back on spiro, but for some reason you have to repeat the AVS (it happens!), you'll need to do the 4-week waiting period while it clears your system again. So, you may want to stay off meds until the results come in if your medical team agrees to this. The one thing I can't stress enough is how important monitoring K is. From the day of my diagnosis last February through three months after my adrenalectomy, I had my K monitored *every week*. When in the hospital for AVS and for surgery, I was unable to leave until my K had stabilized at 3.8 or higher. From other posts I've seen here, other doctors and facilities are not nearly as vigilant about this as mine have been - and I think it's a shame. So, after your AVS, PLEASE make sure they are monitoring your K, and that they don't let you go until it's well into the normal range. A lot of folks here have had post-AVS ER visits because of low K. -msmith1928 Left laparoscopic adrenalectomy 10/13/11 > > I just got a call to schedule my AVS procedure on August 23rd. Guest guest
Max, Have you had an AVS already? What was your experience like? I am being treated at Wash U in St. Louis. Dr. Picus does all of the AVS procedures. My endo at Mayo is comfortable with me getting the AVS done in St. Louis. How much did your BP shoot up before AVS (not on meds)? Did the doctor prescribe any BP meds during this time? Thanks, > > Couple of things: > > . Do they use ACTH during AVS? If not then.see what Dr. Grim says J > > . Is the AVS radiologist experienced? Guest guest
Thank you Dr. G. I did go to them and they answered most questions. However, my endo also advised me to try to talk/communicate with other people that have had the procedure done. The advise I received from my team so far: Exercise Aerobic = good and encouraged anaerobic = not advised with no meds. So no weight lifting, sprinting, basket ball, etc. in his opinion. This is many precautionary he admitted. Potassium It was advised that I continue to eat a high K diet. He told me not to go too overboard though. Just continue to do the V8, spinach salad, etc. Basically DASH. Blood Pressure I have been advised to track my BP each day and call/e-mail him if I get over 160-170/100-90. He might go with a calcium channel blocker if necessary. I am looking for experiences of others who have had an AVS. My doctor did give me guidance. However, he is an athlete with very low BP and no PA. He has never had an AVS. What does the 4 weeks feel like with no meds? Did you go too low on k? Did you have to go to the ER? Any heart problems? Strokes, Heart Attacks? Did you have to start other BP meds? Did you go back on Spiro after the AVS/before surgery? Did anyone lift weights when not on the meds? Thanks, > > > I just got a call to schedule my AVS procedure on August 23rd. This 4 1/2 weeks from now. Guest guest
4 weeks should be fine, see NIH standard protocol in our files: " Aldosterone Causes of Hypertension " NIH did not have any special requirements for me, K+ has always been well within the normal range. You should be ready to travel and play golf. (Use it as an excuse if you have a bad round, they will never know!) NIH scheduled my AVS for Thursday afternoon and discharged me on Friday afternoon. I flew from land to Vermont unattended - no problems. As for delaying the surgery, I see no problems with that, they should restart the Spironolactone once they determine the AVS is good (about a week). In fact, at least one doctor I know says you could delay a lifetime! > > I just got a call to schedule my AVS procedure on August 23rd. This 4 1/2 weeks from now. Guest guest
I have a 2cm adenoma in right adrenal. I don't know about the " plumbing " there but think I remember the testis " drops down " from someplace in the male fetus and have questioned a couple of doctors about that. Nobody had a clue. I hadn't thought of it as if it was obstructing something but it is a good point. I'm starting to work on a review to go over with NIH now that I have talked with all the local doctors and will include that. We didn't talk much about it at NIH. They labeled it as " micro-penis " and charged it off to OSA! I now believe some of my problems are related to the long term effect of Agent Orange since I found that study that found it affected Testosterone. The PA that did my last review of DM side effects suggested I should put in for PA and let the VA prove they weren't related. > > > > > > > I just got a call to schedule my AVS procedure on August 23rd. Guest guest
a, I posted something a while ago where they found they could safely lateralize w/o stopping meds but false or undetermined results needed to be further checked w/o meds. Guest guest
, with your former anxiety issues, have they checked your cortisol levels for subclinical cushing's? They are finding this more often than they once thought. A late night cortisol test is most likely to find it. > > > > I just got a call to schedule my AVS procedure on August 23rd. I am wondering if my AVS is conclusive and I am a candidate for surgery, if it hurts anything to postpone surgery after AVS. My plan would be to go back on the Spiro after AVS and have surgery in December if I am a candidate. Any disadvantages to this? Any danger in this? Any advantages to doing surgery right away? > > > > > > Risk of Stroke/Heart Attack > > > Before I was diagnosed I had crazy, out of control, anxiety! The Spiro and exercise has eliminated this to the point that my former psychiatrist (now friend) told me I didn't need to visit her anymore after on the Spiro for 3 weeks. My attitude changed and anxiety subsided. Well, I have to admit that I am having some anxiety issues with being off the meds for 4 weeks. I am wondering if my BP will sky rocket. I am wondering if I am going to have a heart attack while running, lifting, or playing basketball. > > > > > > Thank you in advance for anyone who has experience dealing with the weeks leading up to AVS. Guest guest
Dr. Grim, Aldo was not tested at that time. However, the low Dex results 0.04 prompted my doctor to test me for GRA. My blood work was sent off to a study out of town. If I remember correctly this was Yale?? Anyway, I received the results back that I do not have GRA. No further study for cushings was done. > > > > > > > > > > I just got a call to schedule my AVS procedure on August 23rd. This 4 1/2 weeks from now. Before everyone lets me know about the 6 weeks off spiro, the Wash U/ protocol is to wait 4 weeks. My endo at Mayo did not seem to think this would be a problem and told me that many publications state 4 week waiting period. > > > > > > > > > > Anyway, I am wondering what are the rules or tips to live by during the weeks leading up to the AVS? I lift weights off and on and play basketball after work throughout the work week. Is exercise off limits without meds? Do I need to take it easy? What have others been told?
