Guest guest Posted July 20, 2012 Report Share Posted July 20, 2012 Hi All, I went and saw my regular PCP on Monday be/c I had developed a hard lump at the site catheter was inserted, area had become very sore and I noticed a bruise had started to develop. This started over the last weekend, so roughly 4-5 days after AVS done. He said he didn't think it was a clot and was probably normal healing process, but that he also had zero experience w/ the procedure. Now it's Friday and I noticed a " bruise " about three inches down my leg from where cath was put in and a bruise has spread across my pubic region from above the cath site as well. Still have hard lump at cath insertion site although not as big or painful. Some intermittent pain extending down my right leg (cath leg) and still can't wear pants as it's uncomfortable to have anything rub against cath site. Is this just normal? I don't know who to ask, as my regular doc made it clear he doesn't have a clue about it and I don't go in for results from AVS until next Friday 7/27. To put it into context, I'm one of those people who had no pain meds post knee surgery, had two kids with no pain meds or epidural--so I've been quite surprised by my level of discomfort given what people have said from this site about their AVS experiences. They are experts in problems after AVS in their shop. Pain ans hard place with something that looks like a bruise sounds like cellulitis. I would go to the ER and make sure it is not a blood clot. My doctors told me it would be normal to have a bruise the size of a quarter, no larger than a half dollar, and a hard lump no bigger than a pea at the site for up to 5 days following. Anything bigger or longer time frame I was supposed to call in ASAP. My lump was smaller than a pea and bruise about the size of a dime,. I was expecting my bruising and pain to be more severe since it took nearly 2 hours to complete my AVS...so it seems like something is definitely "off" with having such a large bruise and pain. I had very minimal pain...only slightly sore if I pressed against it and unnoticeable with clothing or bandages rubbing against it. I agree with that you should go to ER. A potential blood clot or infection is nothing to mess around with. P.S. Be sure to have Dr. Grim review your AVS results. He found an error in mine that was missed by my docs. In fact I saw an Endo and two psychiatrists this week ($96.48 each with travel). We doubled the strength of eplerenone. We doubled the strength of the testosterone patch since 2 months of treatment has shown NO improvement. We radiated the back one more time looking for why I still have back and testis pain. (Only thing found was the kidney stone that has been there since at least 2005.) It has been reviewed by 5 different doctors on 3 different occasions and declared NOT THE PROBLEM! I Refused to let them try again unless they do an exploratory and while they are there snip that adrenal - that will solve the problem! How many IR's treat and monitor blood clots every day? Since the IR is probably not available on w/e should she wait 60 hours? I'm not saying the IR should not be made aware of it but But I feel a hospital emergency room is the best place for treatment and that should happen NOW. Guest guest Posted July 20, 2012 Report Share Posted July 20, 2012 As someone who has had AVS twice, I'm inclined to agree with - this sounds so far from " normal " that you should get it checked out. I bruise very, very easily, yet after my first AVS I had barely any signs of bruising at all - just maybe a quarter-sized spot on each side and no pain at all. The second time, one side had a much larger bruise - about 3 " or so - but again, no pain. I had heard a couple horror stories about post-AVS pain and asked my IR about it in advance, and she said it was pretty rare (but didn't go into specifics about what would cause it). Is the pain extending down your leg in the front, or in the back? After my first AVS I got really alarmed when I had some intermittent shooting pains down the back of my leg a couple days after the procedure. When we have data that using doing X always that is ALWAYS works then we do not need to do trial by error.Proper testing is always good assuming there is excellent data on the accuracy and reliability of the TEST in predicting good response to therapy.I know of no drug or treatment that is not really trial by trial in an INDIVIDUAL.Well take that back. Do not recommend you jump out of an airplane without a parachute. There has never been a randomized trial that I know of this ever being done. Still a rare person does survive. That is the Dr who did the deed and knows what he did and what his complication rate has been. Suspect the ER Dr has have very little experience with this and should call the IR.I would also listen over it for a bruit to make sure it was not a AV fistula. Some intermittent pain extending down my right leg (cath leg) and still can't wear pants as it's uncomfortable to have anything rub against cath site. Is this just normal? I don't know who to ask, as my regular doc made it clear he doesn't have a clue about it and I don't go in for results from AVS until next Friday 7/27. To put it into context, I'm one of those people who had no pain meds post knee surgery, had two kids with no pain meds or epidural--so I've been quite surprised by my level of discomfort given what people have said from this site about their AVS experiences. Both the Phsyco Docs I saw this week agreed with my assessment on how the excess cortisol was potentially worsening my depression. My concern is that you and most others only look at BP to judge success. Potentially yes so trial by error.Seems to me side effects of spiro should have been picked up sooner and Rx changed earlier. I ask pt at every visit about breast pain and then check the breasts. If any problems recommend changing to eplerenone. As for picking up side effects sooner, THAT IS BULL SHIT! You were aware of it as it was happening and because there was no pain you did NOT recommend switching. My K had dropped to 2.7 from 3.6 prior to AVS. So attending Dr called my nephrologist who recommended IV K and increasing Inspra to 50mg. Realized pain in legs and weird fatigue was similar to pre-Inspra days. So, was in hospital until late today when I convinced them to let me go home when levels went up to 3.4. Figured if I went months at 3.1/2.7/3.0, then 3.4 was pretty good. Doctor agreed (!!!) and let me go (I figure I could get myself a lot healthier resting at home eating my high K diet than getting more exhausted in the hospital...). See nephrologist on Friday of this week. So what's everybody's guess---K levels plummeted because of the AVS? No one monitored it afterward--not immediately or anything. I got a copy of labs from hospital:Chloride 114carbon dioxide 22BUN 5Creatine .47Glucose 90Calcium 8.3Magnesium 1.9WBC 8.4RBC 3.82 HgB 11.5Hct Why would RBC, HgB, and Hct be so low? And no one did any urine samples even though I told them repeatedly about adenoma, low K, high aldosterone history. Diastolic pressure was usually around 89/90, Systolic around 139. Hmmm....Thanks for recommendation to go to ER--it made me find the number for the Dr who did the AVS and she recommended I go to ER too. So the bruising still remains a mystery. At UCLA where I had my AVS's and surgery done, I was told that it was their protocol to not release a patient until their K came up to at least 3.8.
