just a thought but when you said “crying, grabbing, pinching, kicking” this could be simply down to inflammation – I remember feeling like that several times just before coming down with a flu... Feeling week and shaky but not really officially ill yet, and very irritable with pins and needless all over... Several times what really helped me was fresh carrot juice with raw garlic in it, lots of vit C and later some ibuprofen – does this sound like something he would take (carrot and garlic juice I mean). Failing that maybe ibuprofen and whatever else you can think of that lowers inflammation for you/him. Stay away from echinacea.

It comes and goes though, we may get a couple of days without any episodes but then some days he has been crying, grabbing, pinching, kicking for hours on end. Whereas in the beginning they seemed completely out of the blue they mostly happen for a reason now (which could be anything from someone changing the channel on the tv to not being able to go out in the car (we were snowed in for a few days) to not being able to fit all his toys in the place he wants them to be.He is tense nearly all the time but can control it better some days than others. We are keeping a log, trying to find a pattern, just started a 10 day course of diflucan combining with bowel clearout will see if that helps. Also have an appointment with the paediatrician on Monday to see if there is anything medical we are missing (I think he is getting headaches as massaging his head seems to help)and to see what our mainstream options are.
