The company was founded by an HIV+ man, and customer service is very helpful if you have any questions. Sizes go up to XXL, which it claims fits a 45" waist, so you should have no trouble finding something that fits. I own a pair, although I must admit I almost never wear them. They're pretty expensive, they feel too wierd to me, and my partner laughs at me when I wear them. The padding didn’t seem to be where I needed it. They’re also quite heavy, which means that they might be very uncomfortable during hot weather.
