With Tommy some of theis stuff ends up being kind of ongoing...like the hair

cut difficulties. He can't get a hair cut every day, some especially when he is

anxious about other things in his life, I see more of his anxiety driven

explosiveness surrounding hair cuts. It is really hard when the feared thing

doesn't happen too often!

Thanks, . I actually thought of that last night - the possible

actually did this with a hair cut a while back (fear of scissors cutting him).

The

dentist fear seems pretty well settled in him - not the actual dentist but

his utensils. The reward suggestion is certain worth a try though.

Oh, and did I mention - his side of the car is the right side - LOL. This

I very much dislike the dentist. I always try to do whatever I can to

not need to go(that actually includes waiting too long) I am not a fan

of Dr's in general. Years of misdiagnosis will do that (both physical

and emotional) I am glad that your check up wasn't too bad. Do you

have a soft bristle toothbrush?

I used to brush my teeth too hard for years. I now use an electric

brush and just move it around gently. I used to think that brushing

my teeth hard and for a long time was good - didn't realise I was

doing any damage, but hey I still got my teeth :-)

all of my PID kids see the dentist regularly, and he has not as yet

asked us to premed at all. I have asked, and if circumstances change,

I always ask again--like, when Brennan was neutropenic, I called again

and asked. I also asked their ID dr last time we saw him and he said

they shoudl be fine.

The only thing they have done differently, is when Brennans ANC was

very low, they were concerned about scraping. They didnt do that for

that visit, but they did everything else and esp the flouride.

Hmmm. I should have said none of the kids are anxious about the dentist.

Charlie is anxious on other things. :-)

Thanks for the input ladies!

Infection risk. Sorry I should have been more clear.

None of the kids are very anxious...oddly enough. :)

Dayna - premedicate for infection risk or anxiety?

Hi everyone! I'm wondering if anyone can share their experience with me

about dentists. All three kid are going in for a check up, and I believe

Kate has a cavity. Does anyone premedicate? What do you use? I seem to

remember a discussion along these lines a few year ago. Charlie has seen a

dentist, but Kate and Tom never have. Charlie only had an exam, and didn't

need anything. Any ideas?

Thanks so much!

We only premedicate prior to tooth extractions or dental surgery/oral

surgery. Usually a dose of abx 2 hours prior to the surgery/extraction and

then one dose several hours later. We do not use abx for routine

cleanings. I know heart patients need to have this done, though...

Our dentist wanted Olivia premedicated because she has a port. He said he likes

all kids that have an " indwelling " objects to be premedicated. Our youngest

Sydney also has some immune problems (not as bad as Olivia) and she does not

need to be premedicated. Because of Olivias allergies she takes Clindamycin 1

hour prior to the cleaning.

We also use clindamyacin 1 hour prior to seeing the dentist for a cleaning.

I've been told in the past that because of 's immune system problem,

and his history of creating abcesses that they don't want to risk scraping

his gums and risking infection, especially with all the bacteria they loosen

that is then floating around his mouth.

Also - has a mild heart murmur, which I understand is also

justification for premedicating.

We sometimes have to cancel dental visits - he has to take Clinda prior to

the visit but can't have been on it during the 30 days prior - and since

Clinda is one of the only abx that are effective for his abcesses we have

had to postpone more than once. They WON'T do a cleaning without the Clinda

for him.

We haven't had any cavities (yet) so I don't know if the protocol would be

different.

Dayna,

We request that Dani have a private room and always make the appt. 1st thing in

the AM. I call ahead and tell the doc that it's Dani coming in and she makes

sure that there is plenty of time to deal with her personality *and* her teeth

-- or she tells me to reschedule. We don't premedicate but our's can't do

antibiotics unless it's an emergency. The doc offers nitrous oxide (??) or some

gas every time, which we don't use but it's a good option. Everyone takes the

usual precautions (gloves, mask, and such). We bring our own sun glasses...to

deal with the overhead lamp. And, give lots of toys, balloons, etc. at the end.

If you can do IVIG with three, you can do anything!

Tim, PreviDent has really worked for me. You probably are not producing adequate saliva. Some medications can dry out your mouth. Cavities and gum disease increase dramatically in those with salivary problems. This is a link to an older post in " The Body " (2003/2004)and directly speaks

about oral health and HIV. I have been using PreVident for dry mouth for over 1

yr. I have receding gums and this treatment helps to reduce deterioration of the

exposed tooth. Many articles are online and are easily found by doing a search

for " tooth decay and HIV "

There is a new product that my dentist's office is advertising to help prevent cavities by strengthening the enamel and they have one for mouths that are dry from medicines too. It is called Mi Paste. Here is a link. http://www.mipaste.com/ I don't have any fillings in my mouth, but I'm thinking of using it just as a prevention aid. The only thing is it isn't for people with milk allergies and I do avoid dairy, so I'm not sure if it will be best for me.

I don't know of any specific link to HAART, though. Can't stress the importance of oral hygiene enough for people living with HIV. Brush, floss, use listerine-type mouthwash. Get regular checkups. Water-pic type cleaning devices are great.JB
