What to do when meds and therapy don't work

i was wondering what the next step is when meds and therapy don't seem to work?

We have tried so many antipsycotics and he just gets worse. We have therapy

every week. We just got an individual options medicaid waiver in dec. He is so

violent with me and out in the community. He was hospitalized for wanting to

kill me and himself. What to do? I am also at my wits end. I am about ready to

have him live somewhere else for awhile. Hopefully today will be a better day.
