A colleague, who I have worked with and who I have also seen as a patient, got

some new medscape (I think that's what she said as I haven't had time to look

for it yet) cme course about this and then contacted me, as she remembered all

about my story. I have to say if I knew about this it never sunk in. It's called

Autoimmune Polyglandular Syndrome I, II, & III. I ask because it - II -fits me

pretty well and there were a few things she knows about that I have discussed

with her before that piqued her memory. I need some opinion/advice if anyone has

it or is familiar with it. Seems there's no real test for it. Hyperaldosteronism

can be a part of this. Any history seeing this Dr G?

Here is some of my history again:

1. I have PA (adrenal)

2. A HUGE one that is so aggravating is I have had for about 5 years now, a

chronic and very bad thrush or fungal infection in my mouth that comes and goes.

I though maybe it was because my teeth were bad until I read this about APGS II

and it could explain it -as this is diagnostic for APGS II. ABX make it beyond

severe, but it shows up whether I want it to or not, and if I get a little sick,

like a bad cold or flu, it just runs rampant, ut seems to be during aphase when

I am feeling more run down. But I do also get fungal infections on my shins, and

my " jock " also and even seomtimes up my belly - that I've scartched the hell out

of. I always have a script of oral nystatin, but I have lately even just used

the antifungal cream in my mouth. A few months back it was so bad I had to

cancel a class because I couldn't lecture. I have a chelitis all the time & I

use the cream on it just to keep it from hurting.

3. Pernicious Anemia (this is the newest dx as my doc had been treating me for

anemia and now we are most certain it is pernicious type (based on my labs and

awaiting some new ones). I am taking the B12 for 2 weeks now and I admit since I

have been I feel a little better. Colonoscopy was good.

4. I have had hyperthyroidism. I have never been low that we know of- and that

they don't have an exact reason for me having at all (no nodules, he didn't

think it's Graves, and my endocrine had settled on subacute thyroiditis - he

believes it's some kind of autoimmune as it flares after I get a bad flu) - he

called it subacute though my symptoms were very severe & mom, 2 sisters have/had

hyperthyroid too.

4. I have a sad history of incredible dental problems. They are really bad right

now, but I have no money. But dentists are a part of my life. I have been

meticulous about flossing and brushing since elemenatry school, but they are bad

nevertheless and since I was 8 multiple dentists and specialists have said I

lack enamel, and I had to do flouride things through the years, but they don't

know why. They had a bunch of theories through the years such as high acid and

reflux causing it, but no one knows - mom (deceased) and same 2 sisters with

thyroid also have severe teeth issues.

....Anyway this is a strong symptom/component of this APGS II. I outbrush my wife

and care for my teeth and she's never had a cavity. I hardly have any teeth

left.

5. I have raynauds - newer only a year or two old, that I am still waiting to

get in with a rheum to see about. My new doc thinks my mouth dryness and some

eye issues and trigemenal neuralgia -like issue are more likely Sjogrens and not

just my medicine that I blame it on so there's maybe that one too. But she said

well let the rheum sort it out.

6. I have had some cortisol fluctuations above and below the norm on labs, but

nothing crazy and no overt signs of cushings or addisons.

My weight hangs around in my older age (45) around 164-166 and I am 5'10 " 1/2 and

only got low in 2009/2010 during my last severe bout of hyperthyroidism wherein

I once lost 17 lbs in single month.

7. I was VERY healthy into my mid 30's and have been incredibly active

athletically (and I am a drumset drummer too) all my life.

8. I have had a heat intolerance that drives everyone in the same room as me

crazy, started about the time PA did in 2002/2002(?) but it is what it is.

Here's the rub. I have likely talked about most of these symptoms I have had,

and there's even more (a trigeminal neuralgia, insomnias, bowel issues (mostly

constipation) and alot of skin rashes that come and go (not vitiligo or

psoriasis) but everything with me started with the PA, insomnias, and s/s of low

K, about '01/02 I think and yet the other things were either masked back then by

chronic low K or came on later. I do not know. I know that the fungal

infections, the anemia, the insomnia drive me crazy. Headaches in the past gone

now with the treatment of my PA. I was a healthy boy, though we didn't get

immunizations (mom just lied to the schools as she didn't want to put up with us

screaming - they weren't against them and I got mine late teens in paramedic

school - though I had measles, mumps, rubella, but no polio. That's the ONLY

immunization we got (no shot is why).

Of course, after the road I have traveled and the mess my life has become

professionally, I wonder and want to know if this " could be it " .....I am still

a patient too and I want my own validation too for this mess I am in :- ). You'd

be happy to know though that I still do plenty of housework for a husband and

dad of 7 kiddos and I take care of the yards, but I just don't change many

diapers, nor do I breastfeed - I poked our 21 year-old when she was first born

as we used clothe diapers for a while and after that I didn't want to as I freak

when I hurt my kids, but I did change a poopy one just today (not on my 21 old

though - my 10 month old who's 10 months today! Were coming up on a year old on

9/06!). So I don't want excuses; just answers.

I don't know. Something beat the snot out of my immune system the past few years

and multiple issues going on.
