The nurse sent me a form to sign that basically said if my kid gets sick I'm all to blame, I filed it away and never sent it back, never heard anything. Well, yesterday I get a letter from her again saying I need to fill out a similar form as part of state law. I'm about to research it, but this is illegal, right? Guest guest Posted August 17, 2005 Report Share Posted August 17, 2005 Debi (and any other TN residents) - Unless there has been a change since 2002, Tennessee Code Annotated Section 37-10-402 (2002) has NO language whatsoever requiring a parent to sign a statement that he/she is accepting responsibility for any injury or illness caused by their refusal to vaccinate. It's just not there unless, as I said, there have been changes to that section since 2002 but the state's website does not show anything new. And IF she manages to find something, YOU call the state board of education and ask them for a copy as well.
