The Royal Commission investigating this tragic event determined that

staph bacterial contamination of the diphtheria vaccine had produced

a

deadly toxin (we know it today as toxic shock syndrome) causing the

death of 12 children. The Commission recommended that an antiseptic

added to multidose vials to prevent bacterial growth.
