I have made a spreadsheet for my son - he's only two so it may not be applicable to your situation but I'd be happy to email it to you if you have excel. I have spaces for his BMs, *good* behaviours, *bad* behaviours, sleeptimes, foods eaten, supplements and I have some of his most obvious pervasive behaviours noted with a score behind them like: Tantruming 1 2 3 4 5 so I just circle the number as to how the day went. I also have the same for nappy rash, vomiting, etc other things that apply to him. I have one sheet for each day (dh prints them out at work, yay) and I try to do a quick fill in each night. If you have a good form it should only take about 5 to 10 minutes to keep a decent diary. I can look back and get a quick picture of an increase in tantrums, biting, etc. It's been quite helpful.
