To relieve a pressure point you can insert the tree with the bump and use leather softener. Another choice is the classic boot fitting of next size up and then shim to fit with thicker socks. Sock thickness and type is the secret of boot wearing. In London where there are supposedly all sorts of old dietzel shops find a Shoemakers Swan. They resemble a long pair of pliers with a ball into the hole end. Hole goes outside the shoe, ball goes inside center over the pressure point and squeeze. Leather softener again. I also went to the flat heel lace up Ropers when I had to go to full time AFO wearing. AFOs I found the straight off the shelf - unmodified AFOs worked best for me. Only about 35 dollars each at the time. Having no spasticity and a normal arch. The custom made of the time were way to bulky and heavy. The top strap on the AFO is what I could not tolerate. After walking or standing this band would cut into my unmuscled skin on bone shins and form a sharp painful linnear pressure area close to breaking down.. I was afraid of skin break down potential. Salvation came when one of those straps came unglued. On a spur of the moment I put on a knee high pair of fly fishing " wader " socks and turned the cuff down over the AFO. This gace around six inches of a stretching comfortable cuff. I have been using this method for around ten years. This plus the lace up roper was the perfect combination for my specific problems.
