Measuring patient outcomes

Is anyone tracking patient outcomes?

I want to set up an efficient tracking system to measure patient outcomes. I

plan to use this information as leverage for renewing a contract that I have

with a bariatric outpatient clinic through a hospital. I would also like to use

the information as a way to market my services to local surgeons.

If anyone else is doing this, I would appreciate your insight on effective ways

you have found to collect data, what specific outcomes you track ( ie ., weight,

BMI , labs, etc.), and how you use this information in your practice.
