Patient's have to follow a pre-surgical meal plan, which is also low calorie/carbohydrate (basically a full liquid diet) any where from 2-4 weeks, depending on BMI. If their BMI is >50, they have to be on the diet (pre-surgical) for 4 weeks. Patients usually lose any where between 10-30lbs on this meal plan. Are other progams requiring an amount of weight loss prior to surgery? Thanks, Wichman, RD Pre-Op Weight Loss Requirements? We then have all pts follow a low kcal, low CHO diet 2 weeks before having surgery for shrinking the liver. We are considering initiating a pre-op weight loss requirement rather than a " no weight gain " policy. Is this something other programs are doing? We do not require a specific amount of weight loss, but our patients do have a visit with the surgeon 1-week pre-op, and if he is not happy with the amount of weight loss, which is typically if they lose 5 pounds or less, he postpones their surgery. We typically see weight loss from 10 to 30 pounds as well and the general assumption is that if someone loses less than 5 pounds, they are not being compliant with the diet. Once they have seen the surgeon (having achieved goal weight or nearly so) and have insurance approval, we put them on a low calorie diet ~ 1000 - 1200 calories for 2 weeks prior to surgery to shrink the liver. We recommend a preportioned meal, meal replacement shakes, 2 fruits, unlimited non starchy veggies and a protein bar.
