Happy Holidays Docs!!

Have a woman in her 50’s with one of the worst cases of AC I’ve ever encountered. ROM in abduction has hovered around 90deg for roughly 10 mos. She’s been getting PT and acupuncture on pretty regular basis for 10 months. She comes to me for neck issues on same side 1/month. Each time, I note no improvement. As a matter of fact it appears she’s beginning to get some compensatory things going on which she’s confusing with an increased ROM. (contorting herself in a way to perform certain tasks such as putting hair up/put shirt on etc)

I’ve sent her to Rebound for surgical consult. They’ve basically told her that a two year rehab is prudent before surgery. MRI: She’s got some calcification of supraspinatus tendon, mild distal supraspin. tendinosis, not tear. Infraspin. intact...... mild AC joint osteoarthritis indicated by capsular hypertrophy

I’ve also discussed platelet therapy and prolotherapy, but apparently some of her docs think that may cause more thickening?

Do any of you have a good suggestion for this woman? She’s very active in her rehab and does a ton of stuff on her own as well. Two years seems like a long time to be inhibited to this degree and frankly, I am skeptical of it simply resolving without intervention.
