our protocol is to give 100 mg IM daily for 3-5 days depending on symptomology (in this case sounds like 5 days is best). Then we recommend oral 100 mg thiamin 2x daily until levels indicate pt is maintaining level. Of note, I have 4 patients who are requiring 100 mg IM weekly to maintain levels and are not able to stay at normal level with any level of oral replacement. Make sure you monitor.....

How much thiamine would you give to someone who is def. in thiamine and how much after repletion would you give to maintain

levels? This is in regards to a Roux-en-y GBP who patient had surgery in 2004 and is now 88 lb. She has persistant N/V with new onset of altered mental status.

Also, how often do you see patients with copper def. after gastric bypass surgery?
