Evaluations of your bariatric programs

Can anyone provide feedback as to how you evaluate the success of your program?

What outcomes do you look at, how do you document them, who assesses/evaluates

the outcomes, what is the protocol you follow for improving poor outcomes, are

all members of the multidisciplinary team notified of the outcomes collected,

where do you keep your outcomes (computer database, submit elsewhere), etc?
