I just use a small dowel that I curl around with my distal fingertips.  I learned this from the venerable Dan Beeson  years ago.  I have several size dowels which I vary per patient size.  Works perfectly and cost almost nothing.

I ordered one of these from Scrip about a year ago, it is a Tepper Wedge or a T-Wedge 2, I'm not sure. It is a little too small for my big hand, and I never sent it back to the company. I've been using a Tepper Wedge since since '96 and truly can't live without it. As Matt mentioned, for some reason they are making them smaller now and difficult for larger hands. I'm glad I got mine when I did (I have one in each room). If you need any pointers when/if you get one, I'd be happy to have you come over to my office.
