I would think of Woll, MD at Rebound for this. If you are worried about an EHL rupture at the tuft it would be treated like a mallet finger. If in the joint space you have a bigger issue. Based on the location and depth of injury, I am concerned that she tore her EHL tendon, she cannot extend her 1st ray (although it's only been 2 days post-injury). I would check out Dr. McClanahan who is a podiatrist and an avid runner.  He is also a strong supporter of chiropractic and treats conservatively  He is in NW Portland. Schneider DC.

He does not do surgery.  I know McClanahan does no surgery but a conservative view up front is always useful in my view.  I like to see if there is a less invasive way to resolve problems before rushing into surgery.  I know that McClanahan will recommend a surgeon if necessary.  I am not so confident that the reverse is true of many experienced docs who have surgery in their bag of tools.
