With all Jan attempts to do it is very likely she simply hasn't had time to look at the bunch containing your email and simply has not seen it yet. Call the office (503-256-1601) and have Jan or Kira email you an attached version of the application which is what I just did for two other doctors in Centeral Oregon..... We are working very hard to clear up any perception of mismanagement that may have been perceived in the past and are trying to improve our communication to the field, as well as improve membership benefits.
