I still use 847.1 with no problem.  I think you have a weird adjuster on your hands. Schneider DCOn Tue, Jul 26, 2011 at 3:23 PM, Abrahamson <drscott@...> wrote:

We used to use these diagnosis codes as needed:

847.0    Cervical spine sprain/strain

847.1    Thoracic  Sprain/strain

847.2    Lumbar Sprain/strain

Then, several insurance carriers asked us to stop using:

847.1    Thoracic  Sprain/strain

And suggested that we use:

739.      Thoracic  Subluxation

Notwithstanding that they are totally different entities, they were happy.

We just had a claim sent back saying the 739. Is too vague so we’re supposed to add more digits.

Would love to use the terribley specific:

719.48   Arthralgia of Thoracic Spine (look at all those digits!)

Anyone have insights into a proper code for Thoracic  Sprain/strain type injuries?

Thanks.

Tell the insurance company to stick it where the sun don’t shine y…..847.1 is the code period to add specificity to the “diagnostic story” note the mechanisms of injury e.g., E812.0 “Motor vehicle collision with another vehicle. Additionally if clinically appropriate describe the other conditions symptoms that may be related to the cervical sprain or complicating factors. As my orthopedic mentor once said the late Dr. “stony” Stonebrink, your diagnosis should tell an accurate story as per what is going on with your patient what is wrong….not just throwing out DX for the sake of reimbursement or “willy nilly.” For example 847.0 Cervical Sprain (Acute, grade I C1-C2) with associated 723.9 Cervicogenic Headaches (Acute, intermittent suboccipital-temporal distribution) and complicated by 721.0 Cervical Spondylosis (Pre-existing, mild C3-C5). Something like that…..your diagnosis must be accurate as possible and tell the story of your patient…… Cheers, Vern Saboe PS: BTW there are indeed four grades of cervical sprain injuries…..we really only treat grade I & II From: [mailto: ] On Behalf Of AbrahamsonSent: Tuesday, July 26, 2011 3:23 PM ListserveSubject: ICD code mania We used to use these diagnosis codes as needed:847.0 Cervical spine sprain/strain 847.1 Thoracic Sprain/strain847.2 Lumbar Sprain/strainThen, several insurance carriers asked us to stop using:847.1 Thoracic Sprain/strain And suggested that we use:739. I didn’t even want to talk about this stuff.

A lot of times, I send stuff to friends pertaining to things like the damage of lifestyle related illness and the enormity of our nation’s debt.

I often get answers like “you’re preaching to the choir.”

I’ve decided that maybe the choir needs a good sermon now and then.

Thanks for the heads up.

We will now wade back into battle with claim delaying cubicle dwellers.
