nutrition care process forms

Does anyone have any pre or post op assessment forms incorporating the nutrition

care process that they would not mind sharing? I have the " ADA pocket guide to

bariatric surgery " which is very helpful but I am having trouble with making it

work with my current forms. I think part of the problem is I am still trying to

understand the nutrition care process as we are currently not using it while

making new forms for computer charting. Any help/advice would be greatly

appreciated. Thanks! Beth Gries
