what info do you turn in for payment of out patient education?

i just got my NPI number and we are starting to see our medicare patients preoping for weight loss surgery and i need some help setting things up

1. first off does anyone have a list of the codes im to turn in or where to get them (or what i am suppose to do with my NPI number)

2. do you use a special form? i was told that my usual nutrition form may not be approriate- that it may be missing info (not sure what info i would be missing so any help would be great)

3. do i need to track time in and time out with my sessions? (i did that in texas and not sure if that is necessary)

any help would be appreciated!!!!!!!!
