post op gastric bypass pt breast feeding (baby 10 days old)

hello

i am having trouble finding breast feeding nutrition recommendations for a post op gastric bypass pt (thank you for the nice article jeanne)

i did find in the files section of the bariatric group pregnancy recommendations and pretty much used that

prenatal mvi

500 mcg b12 daily

DHA 300 mg/day

calcium citrate 1500-2000 mg/day

iron as needed

calories around 1500 and pro needs 1.1 g/kg ibw

okay is this okay for breast feeding ..or what do i need to change..

she is having a hard time producing breast milk at this point..we just advanced her diet to bariatric full liquids (with added protein)

How old is the baby? Breastfeeding before surgery or after -- can't tell from your email. She needs 1500 to 1600 calories for adequate milk production. She should be feeding frequently on both breasts in order to produce additional milk. I will attach a powerpoint that I did. For milk production, she might best benefit from a lactation consultant and then have you oversee her nutrition.

Is she exclusively breast feeding? What gives her the sense that she is not producing enough milk? Is the baby not gaining weight? >well this patient got discharged so no luck giving her more information

she was exclusively breast feeding until she got discharged. she was told to

dump her milk right now. she said she felt that she wasnt producing enough

because she was producing alot prior to being admitted and now shes almost dry

(per pt)

we have no lactation consultants here- we are just a surgical center...i did

provide her with with the article by jeanne..

Hi ,I not sure why she was told to dump her milk (because of medications she is taking?)...If she wants to try to keep breast feeding...she should still pump her breasts to keep whatever milk supply she has. As far as a resource for a lactation consultant...the patient can type into her Google browser lactation consultant and the city where she lives. Lactation consultants are often in private practice. La Leche League is another resource.
