What age did they receive their first cast? How many casts have they gone through? What are their angles now? How old are they now? Just interested in seeing others outcomes from this if any of you have time, I would really appreciate it. I don't mean to ask all girls either, just because I have a daughter, would be interested in boys answers too even, anyone really who has time to share the above info and any additional info. would be appreciated! It was a different type of cast from the Mehta casts. Ian was 15 mos when he got his first cast and I was told to expect him to be in a cast for 12-15 months and then another 6 mos of bracing. My dd was diagnosed w/Scoli. @ 9 mos. with 26o left thoratic curve, measured same @ 12 mos. & progressed to 50o by 15 mos. MRI report was normal & confirmed Idio. Scoli. Now since you're consulting w/M.Mehta I'm assuming it's Mehta cast then (and not Risser?) Can somebody tell me the difference or maybe some websites where I can see the difference in casting.(I know you said to let you know C. if I needed more info on that.. do you know of any sites?) I know I just got that one positive response from a plaster casted patient that wasn't neccessarily referred to as Mehta, I'm a little confused on what the differences are. I know what you're saying about going through this w/the babies, especially while they're so little! I'm not looking forward to it but know it'll be worth every ounce of time & energy if the treatment pays off! I just want to be sure we start off with the right kind of cast/treatment and I'm a little confused on it all. Our ortho is hesitant to give us any kind of " time frame " on length of casting too & says that real agressive curves are sometimes leaving us battling them through all their years of growing & that really makes me nervous to think about this not being correcting for her curve. His idea is of course to correct it but won't neccessarily make any guarantees you know. But from reading Min Mehta's statistical report listed in the files here she falls into the catagory of it taking about 12-24 mos. of casting, her report just states plaster/plaster of paris casts so I was assuming that any plaster cast would be okay & this ortho that we're seeing has the right idea, I'm wondering if that's the case now though. I've been in touch w/his assistant on a regular basis while I'm looking into this and having questions, I emailed her that Mehta report and she said that's the same kind of casting they're doing & it looks similar to the one pictured on the baby.. the only difference being the arm & thigh for leverage which makes sense to me when thinking about it. Those are the lines I was thinking along from seeing how successful the newer Mehta " technic " seems, but was having a hard time w/understanding the difference.. now I have a better idea. My husband is very impressed w/Children's Hospital and our ortho there so I'm having a hard time getting him to understand why there might be better options available that Children's Hospital physicians aren't yet familiar with or atleast practicing. And why I don't want to jump into throwing her in a cast there when we might be wasting the precious time of her infancy growth. He seems to think she's going to be just fine w/what the Dr. wants to do but I just don't want to take the chance if there's a better alternative.. even if it means traveling. However our time is limited, she's already 15 mos. and it basically doubled it's curved state in the past 3 mos. Is Shriner's the only hospitals using this casting technic does anyone know? I'll also check out the other boards too , thanks. I guess now I would need to find out what our closest options for Mehta casting are without wasting anymore time. We actually, coincidentally, are vacationing in Park City from the 8th-15th of this month (flying into SLC) and were planning on getting her casted immediately upon returning. I agree that travel would be worth it if someone can guarantee her success, I would do anything.. inconvenient or no it's not even an issue. So Mehta casting uses a machine huh? Where is Cole at now with his treamtment? I'm wondering if we might be able to set something up while we're out there.. any contact info. or sites you might have to give would be appreciated. Travel is no issue, we'll do anything we have too & have a time share in Park City we can always use for stay if need be. So Lexi's @ 13o right now right? Guest guest Posted January 3, 2006 Report Share Posted January 3, 2006 Yes yes, I'm definitely going to see what they can fit us in for.. & my husband agrees, we'll take advantage of the time we'll be there. Yeah, we've been looking forward to PC too! We have a week in our time share once a year & I'm happy to incorporate some of the time there for this, it's actually strangely coincidental the way it's working out.. like 1 of the only 2 locations in the U.S. incorporating this into their treatment plan happens to be where we are flying to on Sat.?? Could it be anymore convenient? We'll have my 2 1/2 yr. old w/us too but g-ma & g-pa scheduled their own week they have @ a different property to spend some vacation time w/us so they can even help w/her if we need it! Our ortho here kind of explained it to us that way too with guiding the growth, he's definitely one for casting & had a very no nonsense approach to getting her wrapped up, I think it's just a matter of their knowledge & exposure to the way Mehta (and others here now too) are casting, I've got to imagine he's up on recent literature from this past year though? And hearing that Risser is an " older " cast to use & not used as widely anymore is surprising to me that it's what they are using @ S.D. Children's hospital? I have yet to speak w/him about all of this since her appt. last week but will for sure have the chance tomorrow he'll be back in. I've been back and forth with his nurse about everything though & she's been interested in discussing all the concerns I have & any info. I've had to share on Mehta that is being shared w/me.. requesting files be e-mailed etc. He & his staff are great and completely understand us wanting to explore our options for Solana's sake so I know they'll be supportive in working with us regarldess of where we end up starting treatment which is also comforting. (It's been my husband resisting any other ideas than what we're doing which has been hard but I've gathered enough info. that he's starting to come around more, this is all so new to both of us). That's so great the progress you have made w/Cole in only 1 cast, very encouraging, luckily you started early.. I pray we have the same luck! Her hump is barely noticable (except to me of course as well as her rib) and it straightens out nicely when you apply force, but it's obviously getting worse. Guest guest Posted January 3, 2006 Report Share Posted January 3, 2006 Yeah, that's what I said to them like uh, really sorry here, no offense-- I like & extremely respect him & the hospital but am just wanting to make sure that we get this done right & am finding a whole lot of info., positive outcomes, and advocacy on Mehta & not a whole lot on risser, that's for sure. I emphasized wanting to start this right at our last appt. & in verbal honesty held off on scheduling the casting date until I researched it more, in addition to the vacation we had planned in a week & he agreed a week probably wouldn't make or break her correction but we need to figure something out ASAP. We've spoke on the phone & through e-mail several times since & is helpful in answering questions & interested in our situation + any info. I'm finding, she's said they can't possibly take things personally in their line of work & agreed Solana's welfare is the most important thing at hand right now and if anything they respect a parent taking the time & effort to look into this.. especially w/her age. I'm curious to see if he knows anything about Mehta, I've got to imagine so but then again you never know I guess! I was wondering how often she checks these posts, I'd love it if she told me more!! I checked out Bethany's pics & it looks like the exact same kind of table used in risser casting?? Where did you hear risser is the old type of casting used (I think that was C. right?) Just curious because this is the exact casting procedure they do that they refer to as risser casting (or risser cotrel casting) - http://www.srs.org/professionals/resources/risser_cotrel_cast_techniq ue.ppt - which is also listed in the Files area on this site. Now Cotrel, Morel, & Mehta all seem to have the same ideas, as Morel & Cotrel did the study " E.D.F. technique in the correction of scoliosis " , and Mehta & Morel provide the " RESOLUTION rather than control " study in '79, the E.D.F. technique of casting seems to support and be incorporated into the POP casting study done by Mehta & Morel in '79. It seems to me the term risser-cotrel casting is incorporation of the risser (scale) & cotrel's name being involved in the casting term due to fact that E.D.F. IS in fact being incorporated into the treatment as well (being that the E.D.F. technique article/study was in in fact BY him you know?), regardless of what the actual table is called. I'm thinking that the POP casting using the E.D.F. technique (which obviously needs the special casting table that allows those manipulations) is what is most important, wrapping the cast in fiberglass seems like it just aids in it's water resistence.. same with having the gortex liner. Of course I'm not 100% sure on that above but it's kind of the conclusion I'm drawing after going over all this information. Doesn't that make sense to everyone? It seems like the " Mehta " casting you are referring too is really just an incorporation of POP cast using the E.D.F. technique? I LM w/Dr. D'Astous' (sp?) assistant for a but & she used the term " risser casting " in their scoliosis treatment procedure as well when she left a message back for me. I'm going to call her again right now. A 2nd opinion would never hurt, maybe I'll be able to get questions answered over the phone even right now. I'm certainly not thrilled about her having to have a leg and arm in & I want to talk to him about that and all that I've learned to really find out exactly what his plan is & be 100% clear on the technique/equipment that's being used. The bottom line it seems is that casting w/plaster is and important aspect of correction, using BOTH Cobb & RVAD measures in correction and the E.D.F. technique in casting it seems is 2ndly most important.. your hospital is probably up on these 2 things (obviously they are on the plaster & immediate action) and that's why you'r getting great correction. I hope it continues for you too! It's the plaster with the technique that's the main idea in corrective casting of scoliosis patients, as well as Mehta's main idea. The fact that your ortho wanted to brace him & not cast him tells me that they are the ones who weren't knowledgable, on anything, techniques in correcting scoliosis, let alone what risser casting is? Risser (or Risser-Cotrel) Casting, like I said I've found is basically another way of saying a POP cast/jacket using the E.D.F. casting technique... Cotrel (in the Risser-Cotrel casting termanology) is the one who wrote the E.D.F. article/study... having their names together I think shows that risser casts support that (Cotrel's E.D.F.)technique. I posted after this last post too that I did end up getting to speak w/his nurse, I'm sure she's accurate in the termanology they're using for their casting, she was pretty knowledgable on everything they're doing there including the Dr.'s involvement w/Dr. Mehta and what's going on there. I went over everything w/her in extreme detail as did she with me & I clarified all that I've said and my conclusions I've drawn w/her & she said they are accurate and that the Dr. would too, I trust that they are too. They don't refer to their casts as Mehta Casts or Mehta casting however they know what someone means of course when saying that. Do they where you're taking Ian in Erie? Well at any rate I'm so glad to have a clear understanding on everything now & I really appreciate your help & kindness in pointing me in the right directions too! I'm getting together an organised e-mail to send to our ortho going over everything, making sure they've got the right technique & equipment in place there for her plaster case (that in itself is a huge part of the correction and after seeing how difficult it's been for people to get a Dr. to use plaster casting I'm thankful that they're up on it's purpose) including the neccessity of her leg being in I'm wondering what amount of correction is going to worth dibilitating her like that for 3 mos., if it's only an estimated degree or so I don't know if that's worth it you know? But I also don't want to hinder anything, progress or what not. They probably think I'm such a pain, I'm just kidding, I know they don't. Guest guest Posted January 4, 2006 Report Share Posted January 4, 2006 They use a real thin gortex liner between the skin and then the fiber glass outside to ease the idea of getting the cast wet (or washing the baby), probably won't be able to soak in baths or swim or anything. But if we do end up having to do the leg then I like the idea of the gortex liner because It would probably make it easier to clean the diaper area really good between the cast and skin.. bums be out to even think of the idea of getting smelly or what not like somebody else here said, like eeew My original plan & intention was to call & see if they can get us in next week yes, but after kind of having this light bulb hit after looking @ Bethany's casting & realizing that risser-cotrel casting most likely follows cotrel's E.D.F. technique (and confirmed it w/their office) I realized that I wanted to go over all of this w/our Ortho here to see if that the case and if it is I don't see any point in a 2nd opinion all that ways away if we like our Dr. here and he's following the same path as they are there, unnecessary record transfers, x-ray transfers etc. So I'm going to talk to them and decide from there. Guest guest Posted January 4, 2006 Report Share Posted January 4, 2006 That's strange, the only thing I found on risser is that being the scale they assign patients a # for based on some different criteria (age, likely hood of progression, etc.) Maybe older plaster casts were just termed risser casts because of the # scale the used on the patient & when Dr. Contrel & Motrel came out w/their EDF analogy it became risser-cotrel casting technique (like that ppt. in the files section here) incorporating the plaster cast w/Dr. Cotrel's EDF technique I guess. Well I sure hope so about our Dr. here, will know more soon. If not then I'll find some place that is of course & probably ended up traveling to SLC for her treatment! Thank you!! It looks like we will miss you in February! We had some " issues " in December with funding/insurance etc., which we hope have been resolved, but it looks like Siobhan's VEPTR has been pushed to the summer... I am looking into getting a clinic appt in March to have Dr D look at Siobhan again, so maybe we'll see you then?
