Guest guest
This is what I use for pre and post surgery. The two surgeries performed at my center are lap band and RNY gastric bypass. Efficiency is a goal for 2009!! Would anyone be willing to share what they include (i.e. categories, etc.) on their initial visit form and/or share an actual copy? Thank you! Guest guest
I have a pre-op nutrition class or individual appointment where we cover eating behavior, anthropometric data, estimate the individual's protein needs, and I list if the patient has a good/fair/poor understanding. This is for insurance purposes, but also helps me to be sure I cover the basics. Of course there's a lot more covered in the class, but each patient receives a full binder to keep. The individual paperwork that each receives will have my name and contact info, the surgery the person chose, how much protein is needed post-op, and the vitamin regimen required. So, this sheet is for the patient to keep as a reference, along with the binder. I hope that helps. If I'm unclear, let me know!
