Since your patient got pregnant before one

year out the most appropriate goal would be for weight maintenance. Weight gain

is not as important in the first half to two-thirds of the pregnancy but most

important in the last trimester. As long as she is not losing weight and the OB feels the baby is growing appropriately I would not

push the weight gain right now. As I understand it, ACOG is looking at

changing their guidelines for weight gain especially in the overweight and/or

obese female prepregnancy. She should continue eating as she has and

eventually she will likely gain weight as the baby starts to grow. You may

want to have her monitor her weight weekly instead of monthly as the OB does.

I have a patient who is 19 weeks pregnant, about 1 yr

s/p RNY, and

also has PCOS. She has been unable to gain any weight during her

pregnancy. She is 61 " and 180#. She's been eating 3 meals and 3

snacks plus protein drinks. She's even been drinking apple juice.

I suggested she add high calorie things like peanut butter, regular

cottage cheese instead of low fat, smoothies and other drinks that she

can get more calories from.

Any suggestions?
