Guest guest
Hi Docs,I sent a 55 yr old male LBP/Sciatica pt for x-rays, this is the report that came back :FINDINGS: There is 10 mm of anterolisthesis of L4 on L5 and advanced disc degeneration L4-5 and L5-S1. Pars defects are present at L4 bilaterally. No compression deformities are present. Mild anterior spondylosis changes at L2 and L3. I don't remember ever seeing a 10mm before, is that safe or is it surgical consult time? For what it is worth, he came in with a 9/10 pain scale and is down to a 2/10 after some gentle adjustments, but i don't want this fellow out there walking around if he would be considered unstable.Thanks,MattDr. Matt Freedman Chiropractic Physician Pure Life Chiropractic, LLC 1000 Willagillespie Road Suite 100 Eugene, Oregon 97401 (541) 343- 5633http://www.EugeneChiropractic.com/ http://www.EugeneHyperbaric.com/
Guest guest
Hi Matt, Obviously with your skills, he is improving. A surgical consult would be prudent, as the patient needs to be able to make an 'informed consent' prior to it collapsing. No, I'm not projecting that possibility but, with a pars defect already established, a wrong step off a curb could change the picture ... then he may want to know why you didn't 'warm him'. Let him get the consult and make up his own mind. And to do consult with him ahead of time that if he sees drooling on the MD's face, he may want to get a second opinion. Guest guest
Hi Matt, Given the time it takes to develop that, it is most likely stable .... othewise you'd have probably had more difficulty with him than you have obviously had. However comma......a surgical consult would allow the patient 'informed consent' about his possible avenues. Even stable, you and I both know he could step off a curb wrong and have things shift....then he could wonder why you didn't 'warn him'. But, if he gets the consult, advise him that if he see drooling on the MD's face, he may want to get a second opinion.
