Excellent paper, . Glad to see that what is thought to have made a difference in preventing PND was the development of a 'high-quality, therapeutic relationship with the women'. I wonder when health visitors will be able to do this again! Let's hope it's sooner rather than later.

“Universal prevention of depression in women postnatally: cluster randomized trial evidence in primary care”, appears in the journal Psychological Medicine (2010) Vol 40. Cambridge University Press, doi:10.1017/S0033291710001467 at (http://dx.doi.org/10.1017/S0033291710001467)

It is an extension of the excellent PONDER cluster randomized trial, which showed good effects from early diagnosis of PND and follow up by health visitors. The data have been analysed to show whether mothers with a low EPDS score at 6 weeks were equally likely to develop postnatal depression in the intervention and usual care areas, and found that those in the intervention areas fared much better. The figures are significant, but not as robust as we might wish, but the sad thing was that the researchers were not able to get additional funding to chase the additional responses and follow-up needed to strengthen them. If an excellent team like the one that carried out the PONDER trial find it difficult to get relevant funding, there is not much hope for the rest of us! There is a very low priority to prevention and (continuing the debates about worldwide misogyny!) to women's health, as well as to mental health. Even so, a very clear result and well-argued case for universal prevention, through health visiting.
