Yes, all my Dr's. and their addresses and phone numbers are on my paper also, it is so much easier, when someone is not capable of talking or terrible memory, this could be a life saver... My Dr's expect one on each visit also :o)

I also have my meds in my PC that I print out. My rheumy insists on a list at every appointment. I recently put in the list of the diagnosis/surgeries etc. and will print that off. Every time I have to go to a new doc or the ER they ask for it. I know they were happy when I had the meds that they could copy instead of just hand writing. It saves them time and its certainly more accurate than my memory alone :) It probably wouldn't hurt to have a list of doctors and their phone contacts as well. What do ya think? SueH

Good idea!!! I heard something about that on the news this summer... I should put one on my list too, never thought of that...

Oh and an ICE listing in your cell phone, palm, or organizer is a help as well.

ICE stands for In Case of Emergency.

List someone that can be contacted on your behalf should an emergency ever occur.

Good idea... I have my surgeries and medical history on a seperate sheet than my meds and whats wrong list... Both of mine are full pages, so I would have to have something made up to tell them where to find the lists in my purse...

It's actually a great idea to carry around a laminated copy of your meds, surgical history, and doctors.

If you are ever rushed to the ER and unable to answer the questions and no one is there to answer them on your behalf, it can literally be a lifesaver.

Print the list on a bright colored paper and keep it close to your license. If it is too large to put with your license that put a note saying where it can be found.
