Hi Maragaret, forgive my ignorance, I'm not familiar with the regeneration work. What is it , and how will it influence service delivery? I'm just concerned that to roll out FNP from 30 -70 sites , will require a big depletion in numbers and then expect us to identify and target other vulnerable families with limited resources is a tall order .Its starting to seem like a loaves and fishes job to me , kind regards sharman From: margaret@...Date: Sun, 8 Mar 2009 11:45:05 +0000Subject: Re: search for holy grail Hi Sharman Am working in several places where they looked at the FNP and then came to the same conclusion and so did not proceed - this has to be something we raise as part of the regeneration work Margaret Re: search for holy grailHi Margaret I think you have put your finger on the key difference between this proposal and the UNA as implemented in Kirklees; that is a system that capitalises on the health visitor-client relationship, and on the health visitors' professional judgement and interpersonal communication skills. The most impressive thing about that system, is the way it links with the 'whole system' in order to both input information about health needs, and what is done about those needs. I don't think they would aim to use it in the way this PREview instrument appears to be heading: ie, as a screening tool, which will be as much about who does not get a service as who does. However, it is a study that may turn out to be helpful in the end, because the team are good researchers and (with help) may end up making a kind of very clear statement about what is is not possible in an assessment instrument. I recall Browne's work, for example; he was convinced it would be possible to predict which children were at most risk of child maltreatment, and was not daunted by the failure of his first screening tool; he went on to develop a system ('CARE') that required a two-stage assessment and follow-up. In the end, his clear statistics showed that, although it was a good way of working (developing relationships, beginning to understand the particular family dynamics, strengths as well as problems etc) the numbers of false positives and false negatives made it unreliable as a stand-alone screening tool. Perhaps something similarly useful will come out of the York study. I hope someone from Kirklees has been in touch with the York team, as they have (in addition to this job advert) put out quite a wide request for information about current instruments in use.
