A: what do you put on your patient's bill?

Hi , I am in Calif and I use a superbill I copied from my mentor doc here at the office. It includes both ICD 9 and CPT codes that are common. There is also a section for the total bill and doctors signature. I can send you an email copy if you'd like to peak at it.
