Guest guest Posted December 15, 2003 Report Share Posted December 15, 2003 Dear Cheryll, It may be worth considering that from a HV perspective, within our own trust (Ipswich PCT NHS Trust) the postnatal care (particularly Primip's and 'known' vulnerable) of families may commence at least 4 weeks prior to the birth through the Early Intervention Strategy visit we make. This is aimed at being a family health assessment and tries to build relationship with the client whilst identifying strengths and risk factors which may have an impact upon the arrival of the expected child. There has recently been an audit of the use of this tool within the trust. Guest guest Posted December 17, 2003 Report Share Posted December 17, 2003 Cheryll, Having mentioned that there is little in the guidelines about fathers involvement I would like to see some mention about supporting new fathers and encouraging them to be involved in child care as well as supporting the mother. Also - there is nothing in the guidelines referring to the increased domestic abuse risk that we know from research arises postnatally. Family public health should be proactice at this time seeking debt, relationship and domestic problems; particularly as I am aware that in some Trusts these few home visits are all HVs make. I suggest the following additions: section 4.3 4.3.2 iii 'appropritae physical and psychological care of baby at each subsequent postnatal contact' to include assessment of parenting capacity in relation to the prevention of child abuse avoidance of risk of suden infant death syndrome I would add 'avoidance of risk of shaken baby syndrome' (this is almost as common as cot death, occurs primarily in babies under 6 months including some as young as a few days. A third of shaken babies die, many of the remaining are severly brain damaged) 4.3.3 parenting skills - safety issues To include safe head care, avoidance of head and neck injury and shaking. access to postnatal support groups To include awareness of availability of local groups such as for domestic violence and relationship issues. 30% of domestic violence commences in pregnancy Hope that helps .
