Rosemary,

Well as long as you held off as long as possible in reading the instructions...I

guess it's okay that you succeeded.

I haven't run into the problem with people figuring out how to fill it out. I

just tell them it's a continuum scale ranging from one to seven with, for

example, pays attention well ranking 1 and constantly distracted ranking a 7.

Four would be somewhere in the middle--somewhat of a problem.

Since this is a subjective assessment, when I see someone ranking everything 6

or 7--EVERYTHING!--that tells me plenty about that person right off the bat. I

guess I would then go back and ask him to skim through the list and pick the 4-5

items that he considered the biggest problems. Or don't worry about the list

and just work with him on defining training objectives.

Glad you got it working.

Pete

tell people about how to fill it out?

page. I went through and made up a new list putting 4s in for all the ones that

were reported on the first time and the rest came through this time with some

sites listed that did not come up the first time. I know the numbers are not

valid but is there anything else I might be missing in doing this?

Pete,

I would like to be able to see the whole 'key': like on the former Sub assessment, the Process page.

That would help me get to know the symptoms and their respective locations.

Also, I think it's way too long, and i've been meaning to get back to it to see if there are some clarifications and reductions in the questions that could be made...

BUT thanks so much for making the change to "location, location, location'!!!!

Since it's subjective, pretty much any consistent way you/the client choose to

do it is okay.
