6 months of supervised weight management

Hello,

We have added internists to our program and I will now be seeing pts along with the MDs for monthly pre-op visits if their insurance requires 6 mos of wt mgmt.

I am trying to develop simple yet adequate documentation for the RD notes.

Does anyone have anything already in use they would be willing to share?

I will post anything I develop to the list if it is helpful.

If anyone has experience with this component of a med/surg wt mgmt program or is in the process of developing one and would like to email or call me privately, I would welcome it.
