PES statement assessment forms

Hey!

Is anyone using the new Nutrition Care Process in their bariatric or

weight loss assessment form?

No I am not currently and I dread changing to it.

I have started using it. I am not doing the ADI format for charting yet, but am using the PES statements in the assessment part of my SOAP note and goals/interventions in the plan section.
